【摘要】 目的　探讨适合学龄前期和学龄期儿童 (>3岁 ,以下称少儿 )股骨干骨折的固定方法。方法　 2 5例少儿股骨干骨折 ,在C臂X线机监视下行闭合或开放复位后 ,采用单边外固定架固定 ,术后 2～ 4周部分负重 ,骨折愈合后拆除外固定架。结果　经 6～ 15个月随防 ,2 5例骨折全部愈合 ,愈合时间 4～ 13周。下肢无短缩、旋转及畸形愈合 ,无深部感染 ,无再骨折 ,无断钉和螺钉拔出。下肢功能优良率 10 0 %。浅表钉道感染 8例(占 32 % ) ,共 15个钉孔 (占 15 % )。结论　应重视年龄和治疗方法的关系 ,对少儿股骨干骨折应手术治疗 ,单边外固定架治疗少儿股骨干骨折 ,创伤小 ,固定牢固 ,并发症少 ,简单有效更多还原

【Abstract】 Objective To explore an effective treatment method for the pediatric femoral shaft fractures, especially for pre-school age and school age children. Methods 25 cases with pediatric femoral shaft fractures were treated with open or closed reduction and unilateral external fixator under C-arm monitoring. Partial weigh bearing was allowed in 2 to 4 weeks postoperatively. After bony union was found, the external fixator was removed. Results The average time of follow-up was 12 months (range, 6～15 months). All 25 fractures united, and the range of union time was 4 to 13 weeks, average 8 weeks. No serve complication was found regarding leg discrepancy, deformity, refractures and deep infection. No pin breakage or pull-out was found. There were 15 pin track (15%) of 8 patients developed superficial infection. Conclusion The treatment method of femoral shaft fractures in children shoud be concerned in the term of age. The >3 years’ age ones should be treated by operation. Unilateral external fixation is a good method for this kind of fractures. It has advandages of less trauma, rigid fixation, few complication, simplicity and effectiveness.更多还原
