【摘要】 目的　探索彩色多普勒超声的血流指标对前列腺癌的诊断价值。方法　采用彩色多普勒双平面直肠腔内探头检测前列腺 ,重点检测前列腺外周带血流参数指标。结果　前列腺增生 67例 ,前列腺癌 5 8例。前列腺外周带血流 2级以上前列腺增生 5 8% ,前列腺癌 74.13 % ,P <0 .10 ;前列腺增生和前列腺癌的Vmax均值分别是 10 .77cm/s和12 .91cm/s ,RI分别是 0 .71和 0 .82 ,Vmax和RI在两组间有显著统计学差异 (P <0 .0 0 1) ;以RI 0 .75为临界值 ,对前列腺癌诊断的敏感性 87.5 %、特异性 92 .4%、准确性 75 %和阳性预测值 84%。结论　前列腺外周带血流增多 ,RI >0 .75时 ,对前列腺癌的诊断价值明显提高。更多还原

【Abstract】 Objective To evaluate the value of transrectal color Doppler flow imaging (CDFI) and resistance index RI in the diagnosis of prostate cancer. Methods Pathological results of 58 cases of prostate cancer and 65 cases of benign prostate hypertrophy were studied by transrectal color Doppler imaging of peripheral zone.The vascularity on CDFI of prostate was graded respectively on a scale of 0 to 3 and Vmax and RI were measured. Results According to the grades of the vascularity on CDFI was 74.13% in prostate cancer and 58% in BPH(P<0.05). The average values of Vmax was 10.77 cm/s in BPH and 12.91cm/s in prostate cancer, RI was 0.71 in BPH and 0.82 in prostate cancer, (P<0.001). At 0.75 of RI the sensitivity in diagnosing prostate cancer was 87.5%, the specificity 92.4%, the positive predictive value 84%, the negative predictive value 75%. Conclusion Prostate cancer should be highly suspected in case of the high RI level(RI>0.75) on Doppler especially in peripheral zone.更多还原
