【摘要】 目的 :探讨IgA肾病肾活检病理与中医辨证的关系。 方法 :选取资料完整 ,临床诊断为原发性肾小球疾病 ,病理诊断为IgA肾病患者病例 ,参照 1 996年中国中医药学会内科学会肾病专业委员会通过的慢性肾炎 (前期 )中医辨证标准对患者中医症状进行半定量记分 ;参照Memphis分级记分方法对肾活检病理资料进行记分 ;并按“新三因论”假说分别把中医症状分数和肾活检病理分数各分为三类 ,进行方差分析。结果 :中医外感症状值、内郁症状值与病理炎症介质浸润值有正相关性 (P <0 .0 1 ) ;中医内郁症状值与病理组织增生值有正相关性 (P <0 .0 1 ) ;中医正虚症状值与病理组织硬化值有正相关性 (P <0 .0 1 )。结论 :(1 )肾组织内炎症介质浸润的病理改变表现在中医症状上以外邪症状、内郁症状为主 ;(2 )肾组织内组织增生的病理改变表现在中医症状上以内郁症状为主 ;(3)肾组织内组织硬化的病理改变表现在中医症状上以正虚症状为主更多还原

【Abstract】 Objective:To study the relationship between renal pathology and the differentiation syndromes of traditional Chinese medicines in IgA nephropathy.Methods:Patients with primary IgA nephropathy confirmed by renal biopsy were enrolled in this study.Clinical syndromes was assessed by semi-quantitative score system according to the criteria of differentiation of syndromes in chronic glomerulonephritis.Renal pathological damage was assessed by Memphis gradings.The symptom score of traditional Chinese medicine and pathological score of renal biopsy were classified into three categories on the basis of "new three yin" theory.Correlation analysis was performed between clinical and pathological scores.Results:There was a positive correlation between the score of inflammatory infiltration and the score of traditional Chinese medical symptom of Wai -Gan or symptom of Nei Yu( P <0.01).There was a positive correlation between the score of proliferation and the score of symptom of Nei Yu( P <0.01).There was a positive correlation between the score of renal fibrosis and the score of symptom of Zheng Xu( P <0.01).Conclusion:(1)Chinese medical etiology of Wai Gan has a great effect on the pathological change of inflammatory infiltration in the renal tissue.(2)Etiology of Nei Yu has a great effect on the pathological change of proliferation of the renal tissue.(3)Etiology of Zheng Xu has a great effect on the pathological change of sclerosis of the renal tissue.更多还原
