【摘要】 目的 :对骨盆前后环联合损伤各种组合的半盆脱位采用手术治疗 ,减少并发症及后遗症 ,提高疗效。方法 :对 5 7例半盆脱位患者应用前入路骶髂关节复位后 ,再用解剖定位法 ,后路经皮空心双头螺纹钉固定骶髂关节 ,同时用综合法固定前环 ,稳定骨盆。结果 :6 2例半盆脱位 ,除 5例未手术外 ,手术的 5 7例中 5 3例获随访 ,参考 1996年Matta评定标准 :优 4 4例 ,良 5例 ,差 4例。随访期 :3个半月～ 7年 8个月 ,平均 1年 1个月 ,优良率 92 .4 5 ,仅 1例耻骨上切口感染 ,经换药而愈。结论 :应用解剖定位法固定后环 ,同时固定前环 ,简单易行 ,安全可靠更多还原

【Abstract】 Objective:To investigate different combinations of hemipelvic dislocation in combined injury of anterior and posterior pelvic ring undergoing internal fixation to enhance therapeutic effects and get less complications and sequelases.Methods:57 cases of hemipelvic dislocation were reduced by the Sacro-iliac Joint from anterior approach with direct vision,and then fixed by percutaneous nailing with double head hollowed screws applying anatomical orientation.Simultaneously,anterior pelvic ring was fixed by synthetic method,and achieved stable pelvis.Results:57 of 62 hemipelvic dislocations were internally fied by double head hollowed screws.Among these cases,53 were followed up for 3.5～92 months(average 13 months).According to Matta score(1996),they were found excellent in 44,good in 5,and fair in 4.The excellent and good rates were 92.4%.The wound infection of Suprapubis incision occurred only in 1 case,which healed up with 1 month dressing.Conclusion:treated with this scew to fix the posterior pelvic ring fractures and dislocation,simultaneously to fix the anterior pelvic ring by synthetic method is very importence.This approuch is simple and easy to operate.Besides it is sate and reliable.更多还原
