肝体积的计算机测量研究及其临床意义
Study of Liver Volume by Computer Measurement and its Clinical Significance
【摘要】 目的 :探讨用个人电脑及自行开发的软件对肝脏CT断层图像进行三维重建、测量及其临床意义。方法 :术前对 4 6例行原位肝移植的肝硬变门静脉高压症患者和 30例正常对照者的肝脏CT断层图像进行三维重建和测量 ,术后与肝硬变门静脉高压症患者切除的病肝实测肝体积进行对比分析。结果 :对 4 6例肝硬门静脉高压症患者用本方法测得平均肝脏体积为 (983.33± 2 0 6 .11)cm3,同组实测平均肝脏体积为 (90 4 .93± 2 0 9.5 6 )cm3,二者之间呈高度正相关 (γ =0 .96 9,P <0 .0 1) ,本方法测值的平均误差为 8.6 6 %。本法测得正常对照组平均肝脏体积为(12 87.0 0± 96 .18)cm3,与身高、体重、体表面积之间均存在着正相关关系。γ分别为 0 .84 5、0 .933、0 .932 ,均有显著性 (P <0 .0 1)。由体表面积推导正常人群标准肝脏体积公式为 :LV(cm3) =5 70 .1×体表面积 (m2 ) +16 8.8。结论 :运用该方法对肝脏CT断层图像进行三维重建和测量准确性较高。结合本方法测量和运用标准肝脏体积公式计算后所获得的资料 ,能客观地定量评价病肝体积的变化。
【Abstract】 Objective:To study the clinical significance of stereography and measurement of liver volume by computer.Methods:By use of the personal computer and the software of three-dimensional reconstruction and measurement system of liver faultage photograph,the liver volume in vivo was successfully measured and the three dimensional image of the liver rebuilt in 30 noncirrhotic controls and 46 posthepatitic cirrhotics who were selected for orthotopic liver transplantation.They were compared with the reference volume of recipient liver obtained by rmeans of water deplacement after transplantation.Results:The liver volumes of cirrhoitic patients with portal hypertension measured by software and water deplacement was (983.33±206.11)cm 3,and (904.93±209.56)cm 3 respectively.The comparison by means of linear regression analysis between volume measurement on 3D reconstruction software and reference volume showed a nearly ideal correlation coefficient(γ=0.969,P<0.01),the average error being 8.66% .The average of liver volume in controls was (1 287.00±96.18)cm 3,and was positively correlated to the height?weight?body surface area.The correlation coefficients were 0.845,0.933 and 0.932 respectively(P<0.01).The formular derivated by body surface area of unimpaired population was LV(cm 3)=570.1×BSA(m 2)+168.8.Conclusion:The volume measurement of the liver by three dimensional software is relatively accurate.The data of the liver volume obtained by this method and the formular can quantitatively evaluate the change of patients liver volume.
