人类脑胶质瘤P53基因突变与预后的关系
P53 GENE MUTATIONS IN HUMAN GLIOMA AND THEIR CORRELATION WITH PROGNOSIS
【摘要】 目的 探讨P5 3突变在胶质瘤发生发展中的作用及其与临床病理特征和预后的关系。方法 利用聚合酶链反应 -单链构象多态性分析 (PCR -SSCP)及LSAB免疫组化法对已明确诊断的 4 8例脑胶质瘤进行P5 3基因突变以及蛋白表达的检测。结果 4 8例胶质瘤中 2 0例P5 3蛋白呈阳性表达 (4 1.7% )。在高级别的肿瘤 (Ⅲ、Ⅳ级 )中P5 3表达率明显高于低级别 (Ⅰ、Ⅱ级 )的肿瘤 (分别是 6 3.2 %、2 7.6 % ) (P <0 .0 5 )。PCR -SSCP检测发现 17例 (35 .4 % )呈现P5 3基因的单链构象多态性改变 ,均位于 5 - 8外显子 ,突变例数依次为 7(4 1.2 % )、1(5 .9% )、4 (2 3.5 % )、5 (2 9.4 % )。两种方法检测的符合率为 89.6 % (4 3/4 8)。在单因素分析中P5 3突变与预后相关 ,而在多因素分析中P5 3突变与预后无关 ,肿瘤的组织学分级与预后相关性显著。该基因的突变与患者年龄、肿瘤大小、性别无关 ,但与胶质瘤的恶性程度相关性显著。结论 胶质瘤中P5 3突变多发于第 5、7、8外显子 ,该基因的突变与胶质瘤的发生及恶性进展相关 ,与预后无关。
【Abstract】 Objective Purpose To investigate the effect of P53 protein accumulation and P53 gene mutation in the pathogenesis of glioma and their correlation with clinicopathological features and prognosis.Methods Immunohistochemistry (LSAB) and non-isotopic PCR-SSCP techniques were used to detect P53 protein accumulation and P53 gene mutation.Results The positive rates of P53 protein was 41.7% (20/48) in 48 cases of glioma . The expression of the P53 protein in high grade glioma was significantly higher than that in the low grade glioma. PCR-SSCP results showed that im 17 cases abnormal bands were detected in same speciments. The concordance rate of immunohistochemistry and PCR-SSCP was 89.6%. The mutations were located on 5-8 exon. In 7 cases (41.2%), the mutations were located on exon 5,1(5.9%)were on exon 6,4(23.5%)on exon 7and 5(29.4%)on exon 8,respectively.P53 gene mutation was not associated with poor prognosis, age, gender, or tumour location and size. Conclusion The mutation of the P53 may play an important role in the pathogenesis of glioma and it was significantly associated with the grade of differentiation.P53 mutation is not associated with a poor prognosis.
