【摘要】 目的　探讨糖尿病患者妇科围手术处理原则。方法　回顾性分析 72例糖尿病患者行妇科手术的临床资料。结果　全部病例术后 2 4h内血糖均较术前增高。其中全麻及手术时间 12 0min以上对血糖的影响分别明显大于连续硬膜外麻醉及手术时间 12 0min以下者。 18例术后出现一过性尿酮体阳性 ,4例出现酮症酸中毒和低钾血症 ,经胰岛素调节后恢复正常。 72例患者预后良好。结论　糖尿病患者手术麻醉宜选用连续硬膜外麻醉 ,手术时间最好控制在 12 0min。更多还原

【Abstract】 Objective To explore the principles of the perioperative management of gynecological patients with diabetes. Methods The clinical data from 72 gynecological patients with diabetes undergoing gynecological operation were analyzed retrospectively. Results The blood sugar levels of 72 patients increased within 24 h after operation. Effects of general anesthesia with operation time longer than 120 min on the blood sugar levels of patients were more significant than those of epidural anesthesia with operation time shorter than 120 min. Transient positive acetone in the urine after operation was found in 18 patients, and acidosis and hypokalemia were found in 4 patients. Recovery was achieved after regulation with insulin. Satisfactory prognosis was found in all 72 patients. Conclusion The epidural anesthesia is proper for patients with diabetes. Operation finished within 120 min can avoid high blood sugar after operation.更多还原
