大剂量白蛋白对HIE新生儿血液甲状腺激素的影响
Large Dose of Human Albumin Improves the Thyroid Function in Neonates with Hypoxic Ischemic Encephalopathy
【摘要】 目的 研究HIE新生儿血液甲状腺激素的动态变化以及静脉应用大剂量白蛋白对其影响。方法 HIE患儿 76例 ,分为 2组 :大剂量白蛋白治疗组 4 8例 ,静脉滴注人血白蛋白 2 g/ (kg·d)体重 ,连续 3~ 5d ;常规治疗 (对照 )组 2 8例 ,给予常规治疗。在日龄 <4 8h ,7,1 4 ,2 5~ 30d时分别行血清T3,T4 ,TSH检测 ,并与 2 2例正常新生儿比较。结果 急性期HIE新生儿血液T3,T4均明显降低 ,大剂量白蛋白可显著性提高HIE新生儿血液T3,T4水平。结论 新生儿急性HIE时普遍存在血液甲状腺激素水平的降低 ,静脉滴注大剂量人血白蛋白可明显促使血液中甲状腺素水平的恢复
【Abstract】 Objective To identify the changes of thyroid hormine in neonates with hypoxic ischemic encephalopathy(HIE) and the effectiveness of large dose of human albumin the thyroid function.Methods HIE infants were divided into two groups:albuman treated group (n=48),receiving 2g/ (kg·d) human albumin by vein,and control one(n=28). The plasma T3,T4 and TSH were measured within 48 hours,on day 7,14 and on days 25 through 30 after birth.Results The plasma level of T3 and T4 in HIE neonates’ blood was much lower than those in normals ,but it was much higher in albumin treated group than that in control.Conclusions Neonates with HIE suffer from lower lasma T3 , T4 and large dose of human albumin in vein can improve their thyroid function.
