胰液K-ras12密码子点突变联合血清CEA检测对胰腺癌的诊断价值

【摘要】 目的 探讨胰液中K -ras12密码子点突变联合血清CEA检测对胰腺癌诊断的临床价值。方法 采用内镜ERCP从胰管收集 2 8例胰腺癌和 12例慢性胰腺炎的胰液标本 ,聚合酶链反应———限制性片段长度多态性分析 (PCR -PFLP)检测胰液K -ras基因 12密码子点突变并联合血清CEA的检测。结果  2 8例经临床及手术证实胰腺癌患者胰液标本中 ,K -ras突变率为 6 4 3% ( 18/ 2 8) ;血清CEA阳性率为 78 6 % ( 2 2 / 2 8) ;2者均为阳性患者 16例 ( 5 7 1% ) ,其中 6例手术切除胰腺癌为 <2cm的早期胰腺癌。结论 胰液K -ras12密码子点突变联合血清CEA检测有助于胰腺癌的诊断。

【Abstract】 Objective To investigate the value of K-ras mutation in pancreatic juice and CEA in blood for diagnosis of pancreatic cancer. Methods K-ras point mutation was detected using the polymerase chain raction and restriction fragment-length polymorphism (PCR-RFLP) in pure pancreatic juice (PPJ), which were collected from the pancreatic duct under endoscopic retrograde clolangio-pancreatic tography (ERCP). Results The positivity of K-ras point mutation in PPJ was 64.3% (18/28). Positivity of CEA in blood was 78.6%(22/28): Both were positive in 16 patients. And 6 cases tumor were less than 2 cm. Conclusion It suggested that PPJ could be possibly obtained during ERCP, K-ras point mutation in pancreatic juice accompanying by CEA positive indicated that patients were at high risk for the development of pancreatic carcinoma.
