【摘要】 目的　探讨钙拮抗剂对原发性高血压 (EH)患者肾功能的影响。方法　 75例EH患者随机分为非洛地平缓释剂组 (A组 ) 4 0例和地尔硫卓艹 缓释剂组 (B组 ) 35例 ,疗程均为 3个月。观察治疗前后血压 (BP)、血尿 β2 -微球蛋白 (β2 -MG)、尿白蛋白 (Alb)、血尿素氮 (BUN)、肌酐 (Scr)、内生肌酐清除率 (Ccr)、肾小球滤过率 (GFR)的变化。结果　治疗前EH两组血压、血尿 β2 -MG、尿Alb高于正常范围 ,Ccr、GFR较正常值低 ,治疗 3个月后血压明显降低 (P <0 0 5 ) ,血尿 β2 -MG、尿Alb亦显著下降 (P <0 0 5 ) ,Ccr、GFR有提高 (P<0 0 5 ) ,两组间对比差异无显著性意义 (P >0 0 5 )。结论　钙拮抗剂降压效果确切 ,对EH患者早期肾功能损害有保护作用 ,两类钙拮抗剂在降压及肾功能保护作用方面差异无显著性意义 (P >0 0 5 )更多还原

【Abstract】 Objective To investigate the effect of calcium antagonists on renal function in patients with essential hypertension.Method 75 cases of EH patients were randomly assigned to two groups: group A treated by felodipine (Plendil), group B by diltiazem (Dil-SR). Blood pressure and parameters of renal function were measured before and after the three-month therapy. Results The blood pressure(BP)?bloodβ 2-microglobin (β 2-MG)?urine β 2-MG?urine albumin (Alb) exceeded the normal range before the treatment, while the endogenous creatinine clearance rate (Ccr) and glomerular filtration rate (GFR) were lower than normal. There were significant decrease of BP?blood β 2-MG, urine β 2-MG?urine Alb in both groups and increase of Ccr and GFR in both groups after the three-month treatment (P<0.05), There was no significant difference between the two groups (P>0.05). Conclusion Both calcium antagonists could control the BP effectively and had nephron-protective effect in EH patients.更多还原
