围术期缩合葡萄糖氯化钠注射液对凝血-纤溶系统的影响

【摘要】 目的 :考察国产晶花牌缩合葡萄糖氯化钠注射液在围术期对凝血 -纤溶系统的影响。方法 :选择ASAI~II级的择期手术患者 5 3例随机分为A(n =2 7)、B(n =2 6 )两组。经静脉分别输注缩合葡萄糖氯化钠注射液 10 0 0ml(以下简称晶花液 )或乳酸林格氏液 (以下简称平衡液 ) 10 0 0ml,速度为2 5ml Kg hr ,然后用乳酸林格氏液 5 0 0ml hr维持。测定病人的红细胞压积 (HCT) ,红细胞数 (RBC)、血小板数 (PLT) ,测定凝血酶活性 (F .IIa)、纤溶酶原活性 (Plg)。采用凝血 -纤溶动态图仪测定其参数凝固启动时间 (CST)、最大凝固时间 (MCT)、完全纤溶时间 (WFT)、最大凝固程度 (MCE)。结果 :两组HCT分别在输注后即刻与输注后 2h与输注前比较有显著降低 (P <0 .0 5 ) ,RBC数、血小板计数在输注后与输注前比较差异无显著性 (P >0 .0 5 )。纤溶酶原活性、凝血酶活性分别在输注完即刻或输注完 2h表现为逐渐下降 ,但同组不同时点的比较无差异显著性 (P >0 .0 5 ) ,两组间同时点的比较亦无差异显著性 (P>0 .0 5 )。两组病人围术期凝血 -纤溶动态平衡各参数输注前与输注后各时点比较均无差异显著性 (P>0 .0 5 ) ,参数MCE两组均表现为略有下降 ,提示最大凝固时间略有延长。结论 :围术期快速输注缩合葡萄糖氯化钠注射液 10 0 0ml,?

【Abstract】 Objective:To observe the influence of coagulation-fibrinolysis system by polyglucose injection during perioperation. Methods: Randomly select ASA I~II grade patients 53 divided into two groups under epidural anesthesia. Group A(n=27), transfused polyglucose 1000ml ,group B (n=26) transfused equilibrium liquid 1000ml ,both in rate of 25ml/Kg /hr, then both transfuse equilibrium liquid in rate of 500ml/hr. Measured coagulation-fibrinolysis parameters of maximum coagulation time (MCT), coagulation starting time (CST), the whole time of fibrinolysis reaction (WFT), the balance time (BLT), maximum extent of coagulation(MCE) by dynamicgraphy instrument(CFD-III), measured blood plasma thrombin, F.IIa and Plasminogen, Plg , measured RBC, PLT, and HCT at the preoperation, the end of transfuse 1000 ml liquor instantly and 2 hr later respectively. Results: The two groups of HCT decreased significantly after transfuse instantly and 2hr later (P<0.05), RBC and PLT were no significant changes after transfused (P>0.05), F.IIa and Plg were mild lower, but the two groups were compared no significant difference in the end of transfuse instantly and 2 hr later (P>0.05). The coagulation-fibrinolysis parameters were compared with the two stages no significant difference between the two groups (P>0.05 )and the same group (P>0.05 ). Conclusion: Transfusen polyglucose injection 1000ml in perioperation could proliferate significant blood volume , and mild influence to coagulation-fibrinolysis system , take place anaphylaxis infrequency. It could be safely used in perioperation.
