【摘要】 目的 探讨糖皮质激素(简称激素)在治疗严重急性呼吸综合征(SARS)中的临床价值和应注意的有关问题。方法 分析我院2003年4月19日至6月17日收治的21例SARS患者中,经常规治疗病情不能控制而合并应用激素治疗的5例患者的临床资料、治疗经过及转归。结果 应用激素治疗后,5例患者的体温分别于第2～6天恢复正常。呼吸困难于第5～8天明显减轻。胸片于第4～10天病变开始吸收。血氧饱和度于第12～21天恢复正常。5例患者全部治愈出院。结论 在对症、支持等综合治疗的基础上,合并应用激素治疗可以减轻全身中毒反应及阻止肺渗出和肺损伤的过程,提高SARS的治愈率。但需掌握激素应用的指征、时机、剂量和疗程,密切监测病情变化,出现副反应时及时给予恰当处理。更多还原

【Abstract】 Objective To investigate the clinical value and the relevant issues of hormonotherapy of severe acute respiratory syndrome (SARS) .Methods The clinical data, treatment process and end-resulte of the disease in 5 patients out of 21 SARS patients in our hospital from April 19,2003 to June 17,2003 were analyzed, the 5 patients were unable to be cured with the normal treatment, eventually received the combined treatment with hormone. Results After the application of the hormone, the temperatures of the five patients recovered to the normal levels in 2-6 d; dyspnoea obviously relieved in 5-8 d; the absorbing of the affection started in 4-10 d shown in the chest plain films; and blood oxygen saturation normalized in 12-21 d. The five patients were all cured. Conclusion On the basis of a comprehensive treatment,hormonotherapy can relieve the overall toxic reactions of patients and resist the development of the exudation and damage in patients’ lungs, thus improve the possibilities of SARS cure. However, the indication, opportunities,dosage and treatment course should be mastered strictly, and the changes of patient’s condition be closely observed, and the side-effects be treated timely and properly.更多还原
