家兔脂肪肝模型的建立及病理学研究

【摘要】 目的模拟目前人类不良饮食方式建立脂肪肝动物模型 ,探讨脂肪肝的病变特点和病理学量化诊断指标。方法选用 4 0只新西兰家兔 ,分为实验 1、2、3组及对照组 ,实验组采用高脂高糖及含酒精饮料分别喂养 3、6、12周 ,建立不同程度脂肪肝模型 ,对照组采用普通饲料及饮用水喂养。观察不同程度脂肪肝解剖学、病理组织学特点 ,计算肝脏指数 ,并采用真彩色图像分析系统对苏丹Ⅳ染色组织切片进行肝脂肪变面积量化分析。结果实验组家兔形成小泡性为主的肝细胞脂肪变。分别形成轻度脂肪肝 7例 ,中度脂肪肝 10例 ,重度脂肪肝 11例。其中75 %伴发不同程度脂肪性肝炎 ,5 7%有不同程度纤维化 ,11%形成小结节性肝硬化。实验组动物肝指数随造模时间延长而递增 ,与对照组比较差异具有非常显著性意义 ,P <0 .0 0 0 1。图像分析结果显示正常肝脏及轻、中、重度脂肪肝单位测试面积上被测物的面积值依次为 :<10 %、10 %~ 2 5 %、2 6 %~ 35 %和 >35 % ,各组间差异具有显著性意义 ,P <0 .0 0 0 1。结论高脂高糖加酒精喂养可成功的建立不同程度脂肪性肝病模型 ,联合采用病理学和图像分析可更准确的反映脂肪肝的病变类型和程度

【Abstract】 ObjectiveTo establish fatty liver model in rabbits to explore the pathogenesis of human dietary fatty liver changes and study the associated pathological features and feasibility of quantitative diagnosis of fatty liver. MethodsForty New Zealand rabbits were divided into four groups, of which three were experimental groups receiving high dietary cholesterol, high dietary sugar and five percent alcohol in drinking water (group 1, for 3 weeks; group 2, for 6 weeks; group 3, for 12 weeks, respectively). Animals in control group were fed regular diet and water for six weeks. Hepatic gross anatomical and histopathological changes were recorded, and hepatic indexes were calculated. The areas of hepatic steatosis as indicated by Sudan Ⅳ staining were quantitatively analyzed by using a real time color image analysis system. ResultsIn experimental groups, 7 animals developed mild hepatic microvesicular steatosis, 10 developed moderate microvesicular steatosis, and 11 developed severe hepatic steatosis. Seventy-five percent of animals exhibited features of steatohepatitis, 57 % were associated with hepatic fibrosis, and 11 % were complicated with micronodular cirrhosis. Increased hepatic indices were observed with increasing exposure of lipogenic diets, and statistically significant differences were presented between control and experimental groups(P< 0.0001). Image analysis of hepatic tissue sections showed that the positive area in measurement area(Aa) value of control and three experimental groups were < 10 %, 10 %~ 25 %, 26 %~ 35 %, and >35 %, respectively, and the difference between experimental and control groups was statistically significant(P< 0.0001). ConclusionAnimal models of fatty liver disease could be established in rabbits by exposing them to high cholesterol, high sugar and alcohol drink. Combined use of pathological results and image analysis would reflect more precisely the degree and type of fatty liver disease.
