同侧股骨干骨折合并髋部损伤13例
The clinical analysis for ipsilateral hip trauma and femoral shaft fractures
【摘要】 目的 探讨同侧股骨干骨折合并髋部损伤的临床特点和诊治要点。方法 对 1999年 3月至 2 0 0 3年 2月收治的 13例合并同侧髋部损伤的股骨干骨折进行回顾性研究 ,包括髋部损伤类型、早期诊断情况、治疗方法选择等。结果 7例股骨干骨折发生在中 1/ 3,所有股骨颈骨折均发生在基底部 ,而粗隆部骨折类型无明显规律。 11例股骨干骨折和 9例髋部骨折行手术治疗。所有患者随访 10~ 2 5个月 ,骨折均顺利愈合 ,未发现有股骨头缺血坏死表现。结论 高能量所致股骨干骨折患者应常规拍摄高质量骨盆前后位X线片。股骨重建髓内钉治疗股骨干骨折合并同侧髋部损伤是一种较理想方法 ,能同时稳定股骨干和髋部骨折 ,且较少破坏骨折部位血运 ,有利骨折愈合。
【Abstract】 Objective To elucidate the clinical characteristics and therapeutic outline of ipslateral hip trauma and femoral shaft fractures. Methods A retrospective study was carried out on 13 patients with ipsilateral hip trauma and femoral shaft fractures treated at the 6th People’s Hospital of Guangzhou from March 1999 to February 2003(4 femoral neck fractures, 7 intertrochanteric fractures, 1 femoral head fracture, 1 hip dislocation). Results The greater part of the femoral shaft fractures occurred in the middle third(7/13). All of the femoral neck fractures occurred in the base of the neck, but the law of the intertrochanteric fractures was unknown. 11 femoral shaft fractures and 9 hip fractures were treated operatively. The mean follow up time was 14.4 months. Femoral shaft fracture union was within 4 months. All of the hip fracture were healed within 6 months without the evidence of avascular necrosis of the femoral head. Conclusion In case of femoral shaft fracthure resulting from high-energy trauma, an A-P pelivc film should be routingly taken. Reconstruction nail for ipsilateral fractures of the femoral shaft and hip is a good method, and it can stable both femoral shaft and hip fractures concurrehtly and does not distub blood supply of fracture site.
