Ⅰ、Ⅲ型前胶原mRNA在门脉高压症病人脾静脉壁的表达及意义
The expression of Type Ⅰ and Type Ⅲ procollagen mRNA in splenic vein of portal hypertensive patient
【摘要】 目的研究Ⅰ、Ⅲ型前胶原mRNA在门脉高压症时脾静脉壁的表达 ,探讨细胞外基质在门脉高压性血管病变发生中的作用。方法试验组为乙肝后肝硬化门脉高压症病人 18例 ,男 12例 ,女 6例 ,平均年龄 4 8.5岁。住院期间行择期脾切除加贲周血管离断术。选取同济医院同期因外伤性脾破裂急诊入院行脾切除术患者 10例 ,男 7例 ,女 3例 ,平均年龄 31岁。采用逆转录聚合酶链反应检测Ⅰ、Ⅲ型前胶原mRNA在门脉高压症时脾静脉壁的表达。结果门脉高压症时脾静脉壁 (α1)Ⅲ型前胶原mRNA表达明显增高 (P <0 .0 1) ,但门脉高压症病人脾静脉壁 (α1)I型前胶原mRNA表达与非门脉高压症病人相比差异无显著性意义 (P >0 .0 5 )。结论Ⅲ型前胶原及胶原可能是门脉高压症时导致血管构形改建的重要细胞外基质 ,门脉高压性血管病变可能存在着与其他系统血管病变不同的发病机制。
【Abstract】 Objective To investigate the expression of type I and type Ⅲ procollagen mRNA in splenic vein of portal hypertensive patient and to discuss the role of extracellular matrix in the pathogenesis of portal hypertensive vascullopathy. Methods Eighteen hepatic cirrhosis potal hypertensive patients were included in the study. Ten patients with spleen rupture due to trauma without cirrhotic portal hypertension were selected as control group. Type I and type Ⅲ procollagen mRNA in spleen vein vessel were detected by the reverse transcriptional polymerase chain reaction. Results Human (α 1) Ⅲ procollagen mRNA expression in portal hypertensive patients were higher than control group(P<0.01), however, Human (α 1) I procollagen mRNA expression in portal hypertensive patients shows a non-significant difference pattern with control group(P>0.05). Conclusion Type Ⅲ procollagen and collagen may be one of the major extracellular matrix which deposit and resulted in neointimal formation and vascular remodeling in the pathogensis of portal hypertensive vascullopathy.
