血管束植入加缝匠肌骨瓣内固定治疗青壮年股骨颈头下型骨折

【摘要】 目的 探讨血管束植入加带缝匠肌髂骨瓣移植内固定治疗股骨颈头下型骨折的临床疗效及价值。方法 本组 2 3例 ,均为青壮年头下型股骨颈骨折。采用Smith -peterser切口 ,骨折复位内固定 ,将旋股外侧血管束通过钻孔植入到股骨头内 ,取带缝匠肌骨瓣在股骨头颈部开槽后嵌入股骨头内。结果 全部 2 3例获得随访 ,平均随访 2 6个月 (1~ 8年 ) ,骨折完全愈合 ,无一例出现股骨头缺血性坏死。结论 血管束植入加缝匠肌骨瓣治疗股骨颈头下型骨折 ,能有效地改善股骨头血运 ,促进骨折愈合 ,防止股骨头缺血性坏死。对青壮年股骨颈头下型骨折的治疗 ,临床上有继续观察探索价值。

【Abstract】 Objective To investigate the clinical result of treatment of young adult subcapital femoral neck fracture with vascular bundle transplantation and sartorius muscle pedicle grafts assisted with internal fixation.Methods Twenty-three cases of young adult with subcapital femoral neck fracture underwent this surgical technique. The Smith-Petersen incision was use to expose the hip joint, the fracture was reduced and fixed with internal fixation, the vascular bundle of lateral femoral circumflexartery was implanted into the femoral head through the drilled hole, a groove was made between femoral capital and neck, the sartorius muscle pedicled iliac bone graft was impacted into the groove.Result All of 26 cases were followed up. The mean follow-up time was 26 months (12-96 months). All fractures were healed completely without avascular necrosis of femoral head.Conclusion This surgical technique can improve the blood supply of the femoral head and promote fracture healing, meanwhile, prevent avascular necrosis of femoral head, which still remains the value of continuing clinical observation in the treatment of young adult subcapital femoral neck fracture.
