【摘要】 目的　通过对袖状肺叶切除术治疗肺上叶中央型肺癌手术后肺功能的研究 ,探讨该术式对正常肺组织的保留效果。方法　分别对袖状肺叶切除术、常规肺上叶切除术和左全肺切除术共 5 6例肺癌病人的手术前后肺通气功能资料进行回顾性分析。结果　袖状肺叶切除术后肺活量 (VC % )、第 1秒肺活量 (PEV1 0 % )和最大通气量 (MVV % )与左全肺切除者均有明显差异 (P <0 0 1 ) ,而与肺上叶切除者无显著性差异 (P >0 0 5 )。结论　袖状肺叶切除术能最大限度地保留正常肺组织 ,同时其对肺癌的根治性和术后生存率等不逊于全肺切除术者 ,与肺上叶切除者相似 ,确能达到“两个最大”的效果更多还原

【Abstract】 Objective To study the pulmonary function of upper central lung cancer treated by sleeve lobectomy.Methods The pulmonary function(VC%,PEV1.0% and MVV%) of 56 patients with lung cancer treated by sleeve lobectomy,upper lobectomy and total left pneumonectomy from 1992 to 2003 were analyzed.Results The postoperative pulmonary function (VC%,PEV1 0% and MVV%) of 14 patients treated by sleeve lobectomy was significantly higher than that of 18 patients treated by total left pneumonectomy(P<0 001),but have not difference to that of 24 patients treated by upper lobectomy(P>0 05).Conclusions The sleeve lobectomy in the treatment of patients with central lung cancer can maximize preservation of functional pulmonary parenchyma and improve patients life quatity,furthermore,its postoperative radical cure and survival rates are not under that total left pneumonectomy,but similar to the upper lobectomy.更多还原
