氟骨症性椎管狭窄的MRI研究

【摘要】 目的 探讨氟骨症性椎管狭窄的磁共振成像 ( MRI)特点和诊断价值。方法 采用 PHILIPS公司 Gyroscan Intera 1 .5 T磁共振成像系统 ,对 6例氟骨症性椎管狭窄的患者行磁共振成像检查 ,并对其 MRI表现进行分析。结果  6例患者均有明显的氟骨症并有不同程度的椎管狭窄 ,其中脊髓受压胸段 2例、胸腰段 3例、腰段硬膜囊受压 1例 ,受压的脊髓灶状变性 1例 ,脊柱椎骨信号强度减低 5例 ,后纵韧带增厚 3例 ,黄韧带增厚 5例 ,前纵韧带增厚 4例 ,椎间关节膨大 3例 ,椎间盘突出 2例、膨出 2例。结论 MRI能清晰地显示脊髓的受压和变性 ,对氟骨症性椎管狭窄的诊断和疗效评估具有重要价值。

【Abstract】 Objective To investigate the characteristics and diagnostic value of magnetic resonance imaging(MRI) for spinal stenosis with skeletal fluorosis. Methods 6 patients of spinal stenosis with skeletal fluorosis underwent magnetic resonance imaging(MRI) examination on a Philips Gyroscan Intera 1.5T MRI system. The authors analysed the MRI appearances of the patients. Results 6 cases had apparent skeletal fluorosis and spinal stenosis with various degrees. The spinal cord compression included: 2 cases in thoracic vertebra, 3 cases in thoracic and lumbar vertebra, 1 case in lumbar vertebra. 1 case had localized degeneration and necrosis in compressed spinal cord. There were 5 cases with low intensity signals of vertebrae, 3 cases with thickenings of posterior longitudinal ligaments, 5 cases with thickenings of ligamenta flava, 4 cases with thickenings of anterior longitudinal ligaments, 3 cases with enlargements of intervertebral joins, 2 cases with protrusions and 2 case with intumescentia of intervertebral disks. Conclusion MRI can show clearly compression and degeneration of spinal cord and it is very valuable for the diagnosis and evaluation of treatment of spinal stenosis with skeletal fluorosis.
