新生儿呼吸机相关性肺炎病原菌检测及药敏分析
Pathogenic Bacteria Detection and Drug Sensitivity Analysis in Newborns with Ventilator-associated Pneumonia
【摘要】 目的探讨新生儿呼吸机相关性肺炎的病原菌及其耐药情况,从而指导。临床用药。方法 回顾性分析97例新生儿呼吸机相关性肺炎的致病菌及药敏分析结果。结果肺炎克雷伯杆菌为主要致病菌,其次为催产克雷伯杆菌、肺炎链球菌、草绿色链球菌。肺炎克雷伯杆菌及催产克雷伯杆菌对阿米卡星、环丙沙星轻至中度敏感,对头孢曲松、先锋必、头孢呋新及氨曲南较敏感,均对亚氨培南敏感。肺炎链球菌及草绿色链球菌对阿米卡星、环丙沙星中度敏感,对泰能较敏感,均对万古霉素敏感。 结论应加强NICU病房的管理,接触病人前洗手,对VAP病人及时做呼吸道分泌物培养及药敏试验,指导临床用药。
【Abstract】 Ojective To detect the bateria and the bacteria sensitivity in newborns with ventilator- assorcated pneumonia for treatment. Method To analyse retrospectively the result of sputum culture and bacteria sensitivity in 97 newborns with ventilator-associated pneumonia. Results Klebsiella pneumonia was the most common bacterium which caused neonatal ventilator ssociated pneumonia, then klebsiella oxytoca, streptococcus pneumonia and green streptococcus. Klebsiella pneumonia and kleb- siella oxytoca were mildly to moderately sensitive to ciprofloxacin amikacin. They were sensitive to cefuroxime,ceftriaxone,cefoperazone ,aztreonam and tinem. Gland positive bacteria were mildly to mod- erately sensitive to amikacin and ciprofloxacin,they were sensitive to tinem and vancomycin. Conclu- sion washing hands should be emphasized before touching the babies ,to the babies with ventilator- associated pneumonia sputum culture and drug-sensitivity be done at time for treatment.
