超声乳化和人工晶状体植入术治疗高度近视白内障
Phacoemulsification and artificial intraocular lens implantation in the treatment of cataract with high myopia
【摘要】 目的:探讨高度近视白内障患者超声乳化的手术效果和术后角膜地形图及屈光状态的变化。方法:对50例(56眼)眼轴长度超过26.0mm的高度近视白内障患者进行超声乳化手术,术中植入较低度数或负度数的人工晶状体来矫正原有的高度近视。随访6~12月,对临床效果和屈光状态进行观察。结果:超声乳化术后1,3,6,12月视力、矫正视力、球镜、散光、角膜地形图simk值各自比较无明显差异(方差分析P >0.05);术后6月平均视力为0.40±0.24,矫正视力达到0.63±0.29,≥0.5和≥0.8的视力百分率分别为41%和15%,≥0.5和≥0.8的矫正视力分别为79%和32%。术后6月,屈光状态球镜为(-1.02±1.41)D,散光为(-1.35±1.70)D,在±1.00D之内球镜和散光眼分别为56%和35%。矫正后≥0.5和≥0.8的视力,平均百分率分别由37%,18%上升到77%和36%。结论:超声乳化治疗高度近视白内障术后角膜形态Tel押029-82245172Email押IJO.2000@163.com稳定性良好,可获得较好视力。由于植入人工晶状体无调节功能和测量误差,部分患者存在一定程度的屈光不正,需配镜矫正远或近视力获得最佳视觉效果。
【Abstract】 AIM: To observe the clinical effects of phacoemulsification (PHACO) and intraocular lens (IOL) implantation in cases of high myopia as well as the postoperative changes of cornea topography and refractive status.· METHODS: A total of 50 cases (56 eyes) of high myopia with eyeball axis more than 26mm underwent PHACO and IOL. Clinical effects and refractive status were observed during the follow-up of 6-12 months.· RESULTS: No significant change was found in average acuity visual, corrected visual acuity, sphere refractive power,astigmatism or curvature of cornea topography (simk) at 1, 3, 6, 12 months after operation (P >0.05, F test). The average visual acuity and corrected visual acuity were respectively 0.40±0.24 and 0.63±0.29 at 6 months after operation. The sphere refractive power and astigmatism were (-1.02±1.41)D and (-1.35±1.70)D and there were 56% and 35% sphere refractive power and astigmatism within ±1.00D at 6 months after operation. The percentage of visual acuity and corrected visual acuity of ≥0.5 were 41% and 79%; and those of ≥0.8 were 15% and 32%. The average percent age of visual acuity of ≥0.5 and ≥0.8 were increased from 37%, 18% to 77%, 36% after correction respectively.· CONCLUSION: The cornea formation and refractive status are stable and patients can get better visual acuity after PHACO and IOL implantation. Some patients need low diopters eyeglasses to correct the refractive error because of the no-accommodation of artificial IOL and the measure errors of IOL.·
