【摘要】 目的　总结睾丸肿瘤的高频声像图特征 ,探讨高频超声在睾丸肿瘤诊断和鉴别诊断中的应用价值。方法　回顾性分析经病理证实的 37例睾丸肿瘤的高频超声及彩色多普勒血流声像图。结果　 36例被诊断为睾丸肿瘤 ,敏感度为 97%。本组睾丸肿瘤具有共同声像特征 :睾丸肿大 ,睾丸纵隔声像扭曲或消失 ,有占位病灶。睾丸精原细胞瘤、畸胎瘤、睾丸囊肿和表皮样囊肿具有较为特征性的声像图。彩色多普勒超声显示睾丸恶性肿瘤内部及周边血流信号丰富 ,杂乱 ,动脉血流频谱 PSV5 .1～ 19cm/ s,RI0 .4 2～ 0 .82 ;睾丸良性肿瘤内部无或稀少的血流信号。结论　高频超声诊断睾丸肿瘤敏感性高 ,其声像图较具特征 ,高频超声结合彩色多普勒血流对鉴别睾丸肿瘤的良、恶性 ,具有较高的临床价值。更多还原

【Abstract】 Objective To study the image characteristics of high resolution ultrasonography and evaluate the value of diagnosis and differential diagnosis of testicular neoplasms.Methods The gray scale and colour Doppler sonographic images in 37 cases with testicular neoplasms confirmed by pathology were studied retrospectively.Results Thirty six cases were diagnosed as testicular neoplasms,and the sensitivity of ultrasound was 97%.Enlargement of testises,the mediastinum testises tortuous ordisappeared and occupying lesions present in testis were the general ultrsound appearances of testicular neoplasms.Relatively,the sonographic features of seminomas,teratomas,cysts and epidermic cysts were obvious.Colour Doppler flow imaging demonstrated the malignant masses with abundant and irregular flow in the lesions or along the periphery of the lesions.The indices of flow spectral Doppler sonography were measured:PSV,5.1～19 cm/s;RI,0 42～0 82;on the contrary,the benign masses were hypovascular.Conclusions High resolution ultrasonography is sensitive for detecting testicular neoplasms.Some sonographic features may suggest a certain type of neoplasms.The gray scale sonographic findings combined with colour Doppler can differentiate malignant from benign tumors.High-resolution ultrasonography is valuable in clinical.更多还原
