【摘要】 目的　应用三维超声心动图 (3- DE)评价房间隔缺损患者 (ASD)右心室功能。方法　采用 3- DE圆盘总合法测量 2 2例 ASD(ASD组 )患者右室功能 ,与 2 0例正常人 (对照组 ,CON组 )心功能作了对比 ,并探讨了 ASD患者中的 1 4例手术者 (手术组 )手术前后右心室功能情况。结果　 ASD组 RVEDV、 RVESV、 RVSV均大于 CON组 ,RVEF则明显低于 CON组。同时发现 ASD组 RVEF与 Qp/ Qs呈中度负相关 (r=- 0 .5 3,P<0 .0 5 )。 ASD组手术后 RVEDV、 RVESV、 RVSV均较术前明显减小 ,RVEF较术前明显增加。结论　 ASD患者右心室功能确有减退 ,且与分流量有关 ,手术修补 ASD可以使心功能得到改善并防止心功能持续恶化更多还原

【Abstract】 Objective To assess the right ventricular function in patients with atrial septal defect(ASD)using three dimensional echocardiography(3 DE).Methods The right ventricular function of 22 atrial septal defect patients (ASD group)was measured using 3 DE disk summation method and was compared with that of 20 normal persons(con group).Fourteen of the ASD patients underwent group operation(operation group)and their right ventricular function before and after operation was compared to the contrl.Results The right ventricular end diastolic volume(RVEDV),right ventricular end systolic volume(RVESV)and right ventricular stroke volume(RVSV)of ASD group were all higher than those of the con group,but the rignt ventricular ejection fraction(RVEF)was obviouly lower than that of the con group.In the study,the RVEF of ASD group correlated with pulmonary to systemic flow ratio(Qp/Qs)( r =-0 53, P <0 05).The REVEDV,RVESV and RVSV of the operation group were obviously lower after operation than these before operation,but RVEF was increased.Conclusions The right ventricular function in patients with ASD was impaired indeed and correlated with ASD shunt ratio.Surgical treatment could improve right ventricular function and prevent it from successive deterioration.更多还原
