表皮生长因子与胶原蛋白复合物对小型猪修复性牙本质形成作用的实验研究

【摘要】 目的 :观察表皮生长因子 (Epidermalgrowthfactor ,EGF)与胶原蛋白海绵 (collagensponge ,CS)复合物对小型猪修复性牙本质形成的影响。方法 :选用实验用小型猪牙齿 36颗 ,采用EGF/CS复合物为材料进行直接盖髓术 ,通过组织病理学方法常规制备牙 -颌骨联合切片 ,光学显微镜下观察修复性牙本质形成情况 ,并与氢氧化钙糊剂直接盖髓对照。结果 :小型猪牙用EGF/CS直接盖髓术后 2周 ,穿髓孔周边可见修复性牙本质团块形成 ,团块周围有成牙本质细胞样细胞围绕 ;术后 4周有完整的修复性牙本质桥形成 ;术后 1 2周修复性牙本质桥增厚且结构致密 ,严密封闭穿髓孔 ,桥体近髓腔面形成明显的管样牙本质 ,下方为整齐排列的典型成牙本质细胞。用氢氧化钙糊剂直接盖髓 ,术后 2周可见穿髓孔处有炎症坏死区 ,只有少量钙化团块形成 ,术后 4~ 1 2周逐渐形成完整的修复性牙本质桥 ,但形成速度较慢且牙本质桥厚度较薄。结论 :EGF与胶原蛋白复合物显著促进牙髓细胞分化和修复性牙本质形成 ,是一种有临床应用价值的生物活性盖髓剂

【Abstract】 Objective:To investigate the effect of the complex of epidermal growth factor (EGF) and collagen sponge (CS) on reparative dentin formation in vivo.Methods:EGF absorbed onto CS was applied as a direct pulp capping medication in minipig teeth.Calcium hydroxide paste served as control.Samples from both treatment regimens were collected 2,4,12 weeks respectively after surgery and evaluated by histopathological method.Results:Treated with EGF/CS,dental pulp presented partial reparative dentin formation with odontoblastoid around it after two weeks at the exposure site.There was complete reparative dentin bridge 4 weeks later.The bridge was becoming thick with dense construction in 12 weeks.There were well developed odontoblasts under the bridge that contained abundant tubular dentin.In the calcium hydroxide paste capping group,inflammatory cytonecrosis zone was found in the exposed dental pulp and only a little calcial bodies were formed in two weeks.Complete reparative dentin bridge was formed in 4 to 12 weeks,but the forming speed was not as quick as of EGF/CS treated group. Conclusion:The complex of EGF and collagen sponge significantly induces dental pulp cells to develop into odontoblasts and enhances reparative dentin formation.This study indicates that EGF/CS may have potential clinical appllcation as a bioactive pulp capping mediation.
