【摘要】 目的 :以CDI技术探讨AMD眼血流动力学的变化及观察Stulln滴眼剂对眼血流动力学的影响。方法 :应用CDI技术检测AMD患者 33例 66只眼 (其中萎缩型 5 0只眼 ,渗出型 16只眼和正常对照组 33例 66只眼的眼动脉(OA) ,颞、鼻侧睫状后短动脉 (TPCA、NPCA)及视网膜中央动脉 (CRA)血流。动脉血流指标为PSV、EDV、Vm、PI及RI。其中 8例 16只眼用施图伦滴眼剂前后行CDI检测。结果 :①AMD组 (萎缩型 +渗出型 )与正常对照组比较 ,CRA、TPCA、NPCA的血流速度均明显降低 (P <0 0 1)。②用施图伦后AMD患者的CRA、TPCA、NPCA眼血流速度明显提高 (P <0 0 1)。阻力指数 (RI)亦降低。结论 :①老年性黄斑变性的鼻、颞侧睫状后短动脉的血流速度均降低 ,表明AMD的脉络膜血流灌注不良。②老年性黄斑变性的视网膜中央动脉的血流速度降低 ,提示AMD除脉络膜灌注不良外 ,尚有较广泛的眼血流灌注异常或者为一种继发性的自主调节反应。③施图伦滴眼剂可改善眼血流 ,部分病人视力得到提高更多还原

【Abstract】 Objective:To study the changes in hemodynamic features of orbital Color Doppler Imaging (CDI) in age-related macular degeneration and the changes in retrobulbar circulation after using stulln MONO. Methods:33 subjects with age-related macular degeneration (66 eyes,in which nonexudative AMD 50 eyes and exudative AMD 16 eyes) and 33 age-matched control subjects (66 eyes) were compared in the flow velocities of several retrobulbar vessels (OA.TPCA.NPCA.CRA) by CDI on the items of PSV,EDV,Vm,PI and RI.Among the patient group,8 cases were treated with stulln MONO,the changes in CDI were analyzed. Results:The AMD patients (non exudative and exudative AMD) showed a significant (P<0 01) fall in the velocities of TPCA and NPCA,and the resistive index (RI) was significantly higher in the TPCA,NPCA and CRA.After using stulln MONO,the increasing (P<0 01) in PSV,EDV and Vm of the TPCA,NPCA and CRA were observed;the RI was statistically decreased in the CRA TPCA and NPCA. Conclusions:The results suggested that the choroidal perfusion in AMD is abnormal (Perfusion defects);and that there may be a more generalized perfusion abnormality beyond the choroid.The blood supply of the eyes can be increased and the visual acuities of partial patients can be improved after using stulln MONO.更多还原
