一岁以内婴儿法洛四联症纠治术

【摘要】 目的 总结婴儿法洛四联症 (TOF)的纠治经验 ,以提高婴儿 TOF纠治术的成功率。 方法  1999年 3月~ 2 0 0 1年 11月我科收治 4 3例小于 1岁 TOF患者 ,术前均经二维超声心动图或心导管造影检查确诊 ,均在体外循环下行纠治手术 ,其中 5例行急诊手术。 结果 手术近期死亡 1例 (2 .3% ) ,30例患者随访 1~ 18个月 ,恢复良好 ,紫绀消失 ,生长发育明显加快。 结论 小于 1岁婴儿行 TOF纠治术是安全可靠的 ,对反复缺氧发作的 TOF患者 ,强调早期手术 ;对缺氧发作呈持续状态的患者要考虑行急诊手术。改善手术及体外循环方法 ,加强术后监护是提高婴儿TOF纠治术成功的关键。

【Abstract】 Objective To report experience with primary correction of tetralogy of Fallot in infants. Methods Forty-three consecutive infants younger than one year underwent primary correction of tetralogy of Fallot at our department from March 1999 to November 2001. All patients were diagnosed through two-dimensional and Doppler echocardiography or cardioangiography. Primary correction was performed on all patients under cardiopulmonary bypass. Five patients were operated in emergency. Results There was one operative death (2.3%). Thirty patients were followed up 1-18 months and all were asymptomatic and improved in physical development. Conclusion These data strongly support the early repair of tetralogy of Fallot. It is associated with an acceptable operative risk. The urgent operation is emphasized on patients with severe hypoxic spell. Improving surgical and cardiopulmonary bypass technique and postoperative care attribute to the success of repair of tetralogy of Fallot in infants.
