【摘要】 目的 :评价生殖器疱疹 (GH)患者血清单纯疱疹病毒 (HSV)IgM抗体和皮损HSV抗原检测的临床意义。方法 :用酶联免疫吸附实验 (ELISA)方法对 5 2例GH患者在皮损发作期同时检测血清IgM抗体和局部皮损中HSV抗原。结果 :5 2例患者血清HSVIgM抗体阳性 7例 ,阳性率为 13.4 6 % (7/ 5 2 ) ,IgM阳性组疱疹复发平均间隔时间与IgM阴性组比较 ,无显著性差异 (P >0 .0 5 )。皮损HSV抗原阳性 31例 ,阳性率 5 9.6 2 % (31/ 5 2 ) ,其中丘疹、丘疱疹、水疱性损害阳性率 10 0 % (13/ 13) ,糜烂或浅表溃疡性损害阳性率 6 3.6 4 % (14 / 2 2 ) ,结痂性损害阳性率 2 3.5 3% (4 / 17)。皮损HSV抗原检测阳性率显著高于血清HSVIgM抗体阳性率 (P <0 .0 5 )。结论 :血清HSVIgM抗体检测对GH诊断和判断复发价值不大 ,皮损HSV抗原检测对诊断GH活动性感染具有更重要的临床意义。更多还原

【Abstract】 Objective:To evaluate the clinical significance of detection of serum HSV IgM antibody and HSV antigen in herpetic skin lesions of genital herpes patients.Methods:Enzyme linked immunosorbent assays(ELISAs) were employed to test the serum HSV IgM antibody and herpetic HSV antigen in 52 cases of genital herpes.Results:In the 52 cases of genital herpes, 7 were positive for serum HSV IgM antibody, with a positive rate of 13.46%(7/52). There was no sigificant difference in the time intervals of recurrence between the IgM positive and IgM negative groups( P >0.05). HSV antigens were detected in the skin lesions of 31 patients, with a positive rate of 59.62%(31/52). And among them, the positive rate was 100%(13/13) in papular, papulovesicular and vesicular lesions, 63.64%(14/22) in herpetic erosions and superficial skin ulcers, and 23.53%(4/17) in crusted lesions.The positive rate of herpetic HSV antigen was significantly higher than that of serum HSV IgM antibody( P <0.05).Conclusion:Serologic assay for HSV IgM antibody is not much valueble both in the diagnosis of genital herpes and in the assesment of recurrence of the disease as well.Detection of HSV antigen in the herpetic skin lesions has more important clinical significance in identifying active infectons of genital herpes.更多还原
