3D DCE MRA对主动脉病变的诊断价值

【摘要】 目的 :探讨三维动态造影增强磁共振血管成像 (threedimensionaldynamiccontrastenhancedmagneticresonanceangiog raphy ,3DDCEMRA)对主动脉病变的诊断价值。 方法 :10例主动脉病变 (包括主动脉瘤 5例 ,主动脉夹层 3例 ,假性动脉瘤 2例 ) ,在常规MRI(10例 )和cineMRI(5例 )检查后接受了 3DDCEMRA检查 ,扫描序列为 3DFSPGR(GESigna 1.5TMR/i磁共振成像系统 )。结果 :① 3DDCEMRA较常规MRI和 /或cineMRI多检出 1个主动脉瘤、多显示血管分支受累 2支 ;② 3DDCEMRA显示了全部主动脉夹层的真假腔及内膜片 ,显示血管分支受累较常规MRI好 ;③ 3DDCEMRA显示了全部 4个假性动脉瘤病灶 ,瘤腔及破口显示清楚 ,较常规MRI多显示 1个假性动脉瘤病灶。结论 :3DDCEMRA对主动脉病变的部位、范围、血管分支受累显示满意。在常规MRI检查基础上 ,辅以 3DDCEMRA技术 ,将大大提高MR对主动脉病变的显示和诊断能力 ,可替代创伤性的DSA(主动脉造影 )检查。

【Abstract】 Objective:To investigate the value of three dimensional dynamic contrast enhanced magnetic resonance angiography (3D DCE MRA) for diagnosing aortic diseases . Methods: Ten patients with aortic diseases ,in whom five with aortic aneurysms,three with aortic dissections and two with pseudoaneurysms ,received 3D DCE MRA after imaging with conventional MRI( n =10)and cine MRI ( n =5).3D DCE MRA were performed with 3D FSPGR sequences. Results: ①3D DCE MRA detected one more aneurysm and 2 more aortic branches involved than the conventional MRI and/or cine MRI;②3D DCE MRA showed all the double lumens and intimal flaps of aortic dissections ,and showed the involvement of aortic branches better than the conventional MRI;③3D DCE MRA demonstrated all of the 4 pseudoaneurysms ,and the cavities and the slits of pseudoaneurysms were clearly identified. Conclusion: It is satisfactory to use 3D DCE MRA in showing the location,the extent and the involvement of aortic branches.Combination of conventional MRI with 3D DCE MRA will greatly improve the capabilities of MR in displaying and diagnosing aortic diseases ,which can be used as a replacement of the traumatic DSA (aortography).
