髌骨骨折固定术式选择及生物力学分析
The selection of fixed method for patella fracture and its biomechanical analysis
【摘要】 目的 :分析不同类型的髌骨骨折的解剖及生物力学特点 ,结合临床分析及探讨内固定方法的选择、疗效及应用中需注意的问题。方法 :根据作者近 12年来对 72例髌骨骨折患者的资料分析及随访 ,对3种骨折类型的术式及治疗效果作分析对比。结果 :72例患者中 65例获随访 ,平均随访时间 1.5年。所有骨折均临床愈合 ,无明显移位。以术后不同时期膝关节功能和X线检查综合判断疗效。粉碎性骨折及下、上极骨折组以记忆合金髌骨爪加螺钉固定为优 ,横断骨折组以空心加压螺钉加钢丝张力带固定为优。结论 :应根据骨折的类型选择内固定术式 ,以记忆合金髌骨爪加螺钉固定及中空螺钉加钢丝张力带固定为首选。
【Abstract】 Objective: To study anatomic and biomechanical characteristics of different types of fractures and analysise the choice, curative effects of internal fixation for the treatment according to the clinical practice. Methods: According to the information and records from 72 patients of patella fracture who were cured by us in recent 12 years, analysis and comparision for the treatment of 3 types of patella fracture and the relevant curative effects was made. 65 of the 72 patients were visited regularly on average 1.5 year. All cases healed over without obvious displacement. The curative effects are judged by the fuctions of the patients’ knee joints and the results of X-ray examinations in different periods after operation. Results: Shope-remembering alloy claws with bone screws is the best technique for comminuted fracture and fracture of either part of pateua, and cancellous bone screws with tension band for cross fracture. Conclusion: The operation technigues should be chosen according to the types of fracture. Shape-remembering alloy claws and cancellous bone screws with tension band is the suitable technique for the treatment of patella fracture.
