电泳法测定血清脂蛋白(a)胆固醇及其临床应用
Quantitative determination of LP(a)-C by agarose gel electrophoresis and its clinical application
【摘要】 目的 对一种新型的琼脂糖凝胶电泳定量分析脂蛋白 (a)胆固醇 [LP(a) C]含量的方法进行评价 ,并探讨了其与临床常用LP(a)免疫测定法的相关性。方法 采用HelenaREP全自动快速电泳系统分离血清LP(a) C ,然后结合胆固醇酶染色法测定LP(a) C的含量 ,分析了该法的精密度、准确度和干扰因素及正常参考范围 ,并将该法与临床常用的免疫透射比浊法 (ITA)、免疫散射比浊法 (INA)进行了比较。结果 电泳法测定LP(a) C的批内CV为 4 7%~ 5 3%、批间为 5 8%~ 6 4 % ,检测线性范围为 0 0 5 8~ 1 5 5 0mmol/L ,该法基本不受黄疸、脂血、溶血干扰 ,LP(a) C的回收率 92 2 %~93 1%。电泳法 (Y)与免疫透射比浊法 (ITA) (X1)测LP(a)的回归方程为Y =0 6 35X1+0 0 11(r =0 92 9) ,与免疫散射比浊法 (X2 ) (INA)测LP(a)回归方程为 :Y =0 6 6 7X2 +0 0 1(r =0 94 8) ,参考范围为 0~ 0 2 1mmol/L。结论 电泳法能较完全的分离出LP(a) C带 ,其LP(a) C的测定值与临床常用LP(a)免疫法相关 ,操作简便 ,符合临床常规使用的要求。
【Abstract】 Objective To evaluate an agarose gel electrophoresis for quantitative determination of Lipoprotein(a) cholesterol and its clinical application.Methods Quantification of LP(a)-C was performed by a new agarose gel electrophoresis method that allows the separation of LP(a) by Hellen REP system and the determination of LP(a)-C by enzymatic staining of cholesterol,The results of electrophoresis method were compared with the ones of INA and ITA. The specimens of 135 health men were assayed by electrophoresis for the reference range.Results The inter and intra CV of electrophoresis method at low, middle and high LP(a) concentration specimens were 4.7%~5.3% and 5.8%~6.4%. The linearity was 0.058~1.55 mmol/L. Interference with bilirubin (<348 μmol/L), hemoglobin(<18 g/L) and triglyceride(<10 mmol/L) was neglected. The recovery was 92.2%~93.1%. The values by this method (Y) were closely correlated with those obtained by ITA(X 1) [Y=0.635X 1+0.011, r=0.929] and INA(X 2)[Y=0.667X 2+0.01; r=0.948]. The reference range was 0~0.21 mmol/L. Conclusion The precision and accuracy of LP(a)-C determination by the agarose gel electophoresis and enzymatic staining of cholesterol can meet the clinical demand.
