【摘要】 目的　分析上海社区 2 0～ 74岁人群中代谢综合征 (metabolicsyndrome ,MS)及各组的患病现况及其伴发胰岛素抵抗 (insulinresistance,IR)情况。方法　由 1999～ 2 0 0 1年上海华阳和曹杨社区肥胖及其相关疾病基线调查资料中 ,取有完整血糖、血脂、血压、胰岛素、尿微量白蛋白、尿肌酐等资料的 2 0～ 74岁人群共 2 0 4 8人 (男性 896例 ,女 115 2例 )列入本次分析。代谢综合征的诊断根据WHO(1999)代谢综合征的工作定义。结果　 (1)上海社区 2 0～ 74岁人群MS患病率为 17 14 % ,男性在 4 5岁以上及女性在 5 0岁以上MS患病率明显升高 ,6 5～ 6 9岁患病达到高峰。 (2 )MS中最多见的表现形式是高血糖 (或IR) +血脂异常 +中心性肥胖及高血糖 (或IR) +血脂异常 +中心性肥胖 +高血压。 (3)胰岛素抵抗在MS及各组分的伴发频率分别为 :MS71 6 5 %、糖尿病 6 1 11%、糖调节异常38 2 7%、高血压 35 76 %、血脂异常 38 4 0 %、中心性肥胖 38 39%及微量白蛋白尿 37 98%。结论　上海社区 2 0～ 74岁人群中约 1/ 6者患MS ,胰岛素抵抗在MS及各组分中普遍存在 ,尤以糖尿病及MS为甚。更多还原

【Abstract】 Objective To investigate the prevalence of metabolic syndrome(MS) and its components in individuals aged 20-74 years in Shanghai community. Methods Data were gathered from baseline survey of obesity and its co-morbodities in Shanghai Huayang and Caoyang community from 1999 to 2001. Totally 2 048 subjects (men 896, women 1 152) with complete laboratory data, including plasma glucose, lipid profile, blood pressure, serum insulin, urine albumin and creatinine, were analyzed in this study. MS was diagnosed according to the definition proposed by WHO in 1999. Results (1) The prevalence of MS was 17.14%, being significantly increased in men aged over 45 years and in women aged over 50 years, and among subjects aged 65-69 years the prevalence of MS was the highest. (2) The common manifestation of MS was hyperglycemia or insulin resistance plus dyslipidemia and central obesity, with or without hypertension. (3) The prevalence of insulin resistance was 71.65% in MS, 61.11% in diabetes, 38.27% in impaired glucose regulation, 35.76% in hypertension, 38.40% in dyslipidemia, 38.39% in central obesity and 37.98% in microalbuminuria. Conclusion About 1/6 of the habitants in Shanghai aged 20-74 years had MS. Insulin resistance was common in MS and its components, especially in diabetes and MS.更多还原
