【摘要】 目的探讨与神经内镜应用有关的常见并发症和防治方法。方法回顾分析1998年10月至2002年10月应用内镜手术并具备半年以上随访的病例,选取5个有代表性的病种共500例,疾病种类包括颅内表皮样囊肿、垂体瘤、脑积水、颅内囊肿、脑囊虫。手术方法包括单纯内镜手术、内镜辅助的显微外科、内镜下经单鼻孔垂体瘤切除术。结果单纯内镜手术并发症为术中出血8?240例(3.3%);感染5?240例(2.1%);内镜辅助的显微外科手术中颅神经损伤2?110(1.8%)。三脑室底造瘘的病人中,一过性高热10.2%,一过性精神症状9.1%,头痛6.1%,脑膜炎1%。内镜下垂体瘤手术中,6.7%的病人有一过性脑脊液漏;0.8%有脑膜炎。结论神经内镜手术的并发症发生率较低,并且多数是可以防治的。更多还原

【Abstract】 Objctive This is a retrospective study to identify the common complications and methods about prevention and treatment with neuroendoscopy.Methods The clinical records for500patients,including intracranial epider cyst、pituitary adenoma 、hydrocephalus、intracranial cyst and cranial cysticercus,who have underwent neuroendoscopy between Oct.1998and Oct.2002,at our hospital have been analysed.The methods of operation included neuroendoscopic procedure,neuroendoscopy -assisted microneurosurgery and endonasal transsphenoidal endoscopic pituitary surgery.Results Of the complications found in240neuroendoscopic interventions perfomed at our hospital,hemorrhages during the operation occurred in8patients(3.3%)and infection in5patients(2.1%).The intracranial nerve damage occurred in1.8%of patients with endoscope-assisted microneurosurgery.In patients with third ventriculostomy,10.2%of transient fever,transient mental syndrome in9.1%,headache in6.1%,and meningitis occurred in1.0%.Of the patients with endonasal transsphenoidal endoscopic pituitary surgery,transient cerebrospinal fluid leak found in6.7%,and meningitis in0.8%.Conclusion Complication morbility of neuroendoscopic surgery was low and majority of them could be prevented and cureed.[更多还原
