【摘要】 目的 调查并分析社区糖尿病患者伴发抑郁的状况,探讨抑郁对糖尿病的不利影响,为社区健康干预提供依据。方法 对社区115例2型糖尿病患者和基线状况与其匹配的健康对照进行Beck抑郁自评量表(BDI)的调查,同时以问卷调查法收集糖尿病组的病情基本资-料和治疗依从性情况,并测定空腹血糖(FBG)、餐后2小时血糖(2 hPBG)和糖化血红蛋白(HbAlc)。结果 糖尿病组的BDI评分高于对照组(P<0.01),其中有无并发症极显著影响BDI评分(P=0.003),而性别、年龄、文化程度、经济状况、病程对BDI评分无显著影响(P>0.05);糖尿病患者中非抑郁组和抑郁组的代谢控制、治疗依从性状况及并发症人数在统计学上差异均有极显著性(P<0.01),且抑郁程度越重糖代谢控制越差。结论 抑郁对糖尿病的治疗有着及其不利的影响,在社区范围内建立健全健康干预制度不能忽视个体的心理治疗。更多还原

【Abstract】 Objectives To investigate and analyze depressive state in patients with diabetes in a community so as to assess adverse impact of depression on diabetes and provide basis for health intervention.Methods A total of 115 patients with type 2 diabetes and healthy control group matched for demographic data and therapeutic compliance were selected from a community.All the subjects were evaluated with Beck Depression Inventory ( BDI),and their baseline data were collected by a short self-administered questionnaire,including diabetic history and medication compliance.Fasting blood glucose ( FBG),two hour post-prandial blood glucose (2hPBG) and HbAlc were measured for the diabetes mellitus (DM) group.Results BDI score was significantly higher in DM group than that in control group (P <0.01) and depended on diabetic complications (P = 0.003),but independent of gender,age,education,economic status and course of disease (P > 0.05 ).There was significant difference in metabolic control,medication compliance and number of patients with complications between depression and non-depression groups in diabetics (P <0.01).Glycemic control significantly correlated to their severity of depression.Conclusions Depression had notably adverse effect on treatment for diabetes.Psychological therapy should not be ignored in a community comprehensive intervention plan.更多还原
