2型糖尿病患者急性时相蛋白与糖尿病肾病的关系
Relationship between serum acute-phase proteins and diabetic nephropathy in type 2 diabetic patients
【摘要】 目的 探讨糖尿病肾病不同时期血清急性时相蛋白浓度的变化及其意义。方法 正常对照组 3 0例和 2型糖尿病组 10 0例进入本研究 ,根据尿白蛋白排泄率 (UAER )将糖尿病患者分为正常蛋白尿组 ( 3 9例 )、微量白蛋白尿组 ( 3 0例 )和临床蛋白尿组 ( 3 1例 )。采用数率散射比浊法测定血清C反应蛋白、α1 酸性糖蛋白 (α1 AAG)、铜蓝蛋白 (CER)浓度 ,用快速比色法测定唾液酸 (SA )。结果 ( 1) 2型糖尿病患者血清急性时相蛋白浓度较对照组明显升高。 ( 2 )急性时相蛋白浓度随UAER的增加而增加。 ( 3 )多元逐步回归分析表明部分急性时相蛋白 (α1 AAG ,CER ,SA)与甘油三酯及胆固醇相关。结论 2型糖尿病患者中存在急性时相反应 ,而且急性时相反应的强度与糖尿病肾病的程度存在一致性。
【Abstract】 Objective To investigate the levels of serum acute phase protein in different stages of diabetic nephropathy and their implication. Methods This study included 30 healthy subjects and 100 patients with type 2 diabetes mellitus, diabetic patients were further divided into three subgroups according to their urinary albumin excretion rate (UAER): 39 patients with normal UAER, 30 patients with microalbuminuria and 31 patients with clinical proteinuria. C-reactive protein, α 1-acid glycoprotein (α 1-AAG), ceruloplasmin (CER) were measured with rate nephelometry method, sialic acid was measured with MTT method. Results (1) Acute phase protein concentrations were significantly higher in patients of type 2 diabetes mellitus than those in normal controls. (2) Acute phase protein concentrations were increased with increment of UAER. (3) Some acute phase proteins (α 1-AAG, CER and SA) were correlated with serum cholesterol and triglyceride independently. Conclusion There is acute phase reaction in type 2 diabetes mellitus and the degree of acute phase reaction is correlated with the degree of diabetic nephropathy.
