螺旋CT三维重建在修复颌面部复杂骨折中的应用

【摘要】 目的 探讨螺旋CT三维重建在修复颌面部复杂骨折后的应用价值。方法 对 5 0例颌面部复杂骨折患者作螺旋CT三维重建 ,立体直观地表现骨折的断端、部位、移位情况及骨折端与周围结构的关系等 ,依此准备长短大小与形态均恰当的微型钛板、钛钉 ,并选择合适的手术进路与手术切口。结果 通过螺旋CT三维重建 ,于 5 0例清晰地显示出颌面部复杂骨折的状况 ,指引骨折的复位与坚强内固定术 ,42例牙颌功能与颌面外形获得完全恢复 ,8例获得基本恢复。结论 螺旋CT三维重建能较好地显示颌面部复杂骨折的三维立体解剖形态 ,有助于手术进路与切口的选择 ,和准确地实施坚强内固定术 ,可有效地减少外伤后颌面部的继发畸形。

【Abstract】 Objective To evaluate the clinical application of the three-dimensional spiral CT in the correction of the deformity caused by the complex maxillofacial fractures. Methods A total of 50 cases with complex maxillofacial fractures were checked using the spiral CT with the slice thickness of 3-5 mm. The surface shaded display method was used for reconstruction of the 3D images. The 3D structure of maxillofacial fractures was obtained to guide the operation for rigid internal fixation and for the correction of the deformity. Results 3D CT showed clearly the three dimensional anatomical structure and their relationship of the fractures, 42 of 50 cases got satisfactory occlusion and appearance, and 8 cases of them were also satisfied with the results after the operation with the help of 3D CT. Conclusion 3D CT imaging is very important in evaluating the complex maxillofacial fractures and able to guide the reposition and the internal rigid fixation operation for correction of the deformity caused by the complex maxillofacial fractures.
