肾盂输尿管癌多药耐药指标的表达意义

【摘要】 目的 探讨多药耐药指标在肾盂输尿管癌中的表达及临床意义。 方法 应用免疫组织化学技术检测 5 1例肾盂输尿管癌标本P 糖蛋白 (P gp)、多药耐药相关蛋白 (MRP)、肺耐药蛋白(LRP)和谷胱甘肽 S 转移酶 π(GST π)的表达。 结果  5 1例标本P gp、MRP、LRP、GST π表达阳性率分别为 35 .3% ,39.2 % ,5 8.8%和 4 3.1% ;P gp表达随病理分级升高而升高 (P <0 .0 5 ) ,MRP、LRP表达随病理分级升高而降低 (P <0 .0 5 ) ;MRP与GST π表达明显相关 (P =0 .0 0 1,r =0 .4 5 9)。 结论 LRP是肾盂输尿管癌先天性多药耐药的主要指标 ,MRP与LRP更多地在低分级肿瘤中发挥耐药作用 ,P gp在高分级肿瘤MDR机制中占有较高比例 ;MRP与GST π表达在基因调控上可能存在共同的诱导机制。

【Abstract】 Objective To evaluate the expression of multidrug resistance-related markers in renal pelvic and ureteral carcinoma. Methods The immunohistochemical expression of P-glycoprotein (P-gp), multidrug resistance-associated protein (MRP), lung-resistance protein (LRP) and glutathione S-transferase Pi (GST-π) were examined in 51 patients with renal pelvic or ureteral carcinoma.The correlation between the expression and some clinicopathological parameters was analyzed. Results The positive expression rate of P-gp、MRP、LRP and GST-π in renal pelvic or ureteral carcinoma was 35.3%(18/51),39.2%(20/51),58.8%(30/51) and 43.1%(22/51) respectively.The rate of P-gp positive staining was increased with the advance of tumor grade (P<0.05),whereas the rate of MRP and LRP decreased (P< 0.05).Expression of MRP was significantly associated with that of GST-π (P=0.001,r=0.459). Conclusions The positive expression rate of LRP was the highest and suggests that LRP may play a major role in the intrinsic drug resistance in renal pelvic and ureteral carcinoma.LRP and MRP may be the most important multidrug resistance-related markers in the high grade tumors,while P-gp may be the major one in the low grade tumors. The correlation of MRP and GST-π indicates that there should be a common mechanism regulating their expression.
