用18F-FDG PET显像判断淋巴瘤恶性程度
Predicting malignancy grades with PET in lymphoma
【摘要】 目的 探讨1 8F 脱氧葡萄糖 (FDG)PET全身显像判断淋巴瘤恶性程度的价值。方法 对 32例 (非霍奇金淋巴瘤 2 9例 ,霍奇金病 3例 )治疗前的恶性淋巴瘤患者按病理REAL分型标准划分为低度、中度及高度恶性 3组。其中低度恶性 10例 ,中度恶性 18例 ,高度恶性 4例。所有患者行全身PET显像。图像分析采用视图分析及测量病灶最大标准摄取值 (SUVmax)方法相结合。结果 恶性淋巴瘤患者中 ,低度恶性组SUVmax为 0 70~ 6 5 0 ,SUVmax( x±s) :2 95± 1 72 ;中度恶性组SUVmax为2 34~ 2 5 0 0 ,SUVmax( x±s) :8 72± 5 6 8;高度恶性组SUVmax 为 5 90~ 15 10 ,SUVmax( x±s) :10 30±4 0 5。恶性淋巴瘤患者SUVmax低度恶性组与中度、高度恶性组比较差异有显著性 ,虽中度恶性组与高度恶性组SUVmax在数值上有一定范围重叠 ,但结合影像学改变 ,可以提高诊断准确性。结论 1 8F FDGPET显像能较好预测恶性淋巴瘤的恶性程度 ,为临床提供有用信息。
【Abstract】 Objective To determine whether 18F-fluorodeoxyglucose (FDG) PET imaging can predict malignancy grade in lymphoma. Methods 32 patients (NHL, n=29 and HD, n=3) were divided into 3 groups according to pathological REAL standard as low-grade, middle-grade and high-grade malignancy, and all cases were examined by 18F-FDG whole body PET scan. Visual and semi-quantitative analysis were used, maximal standard uptake values (SUV max) of tumor lesion was calculated. Results In 32 malignant lymphoma patients, the group of low-grade malignancy lymphoma’s SUV max: 2.95±1.72, the group of middle-grade malignancy lymphoma’s SUV max: 8.72±5.68 and the group of high-grade malignancy lymphoma’s SUV max: 10.30±4.05. Conclusion 18F-FDG PET imaging can predict malignancy grade in malignant lymphoma accurately.
