SARS的CT表现及动态变化

【摘要】 目的 探讨严重急性呼吸综合征的CT表现及动态变化。方法 对 60例严重急性呼吸综合征患者进行X线胸片 (已有另文报道 )和高分辨率CT(HRCT)的动态观察。患者在入院时及入院后连续进行CT观察检查 ,间隔 4~ 6d。结果 发病初期进行影像检查者 2 5例 ,CT表现为小片状影像者 2 2例 ,占 88 0 % (2 2 2 5) ;其中类圆形磨玻璃样影 2 0例 ,占 80 0 % (2 0 2 5)。其余 3 5例在进行影像检查时已发病 3d ,病变已经有所进展。 3d后患者肺部均出现多发大片状影像 ,其中呈单一磨玻璃样影者 10例 ,占 16 7% (10 60 ) ;磨玻璃样影合并实变影者 45例 ,占 75 0 % (45 60 ) ;主要呈肺实变影者 5例 ,占 8 3 % (5 60 )。入院 10~ 14d内病变明显进展者 50例 ,占 83 3 % (50 60 ) ;病变范围减少者 10例 ,占 16 7% (10 60 ) ;合并肺间质增生者 4例 ,占 6 7%。结论 HRCT可清楚反映SARS病变的密度、形态和范围 ,以及显示肺间质增生改变。因而 ,HRCT可以用于早期诊断和了解病变的动态变化

【Abstract】 Objective To study the CT appe arances and dynamic imaging changes in clinically diagnosed severe acute respiratory syndrome (SARS). Methods Sequential HRCT examinations at a 4- 6 day interval were performed in 60 patients with SARS. The findings on CT images were analyzed. Results HRCT was performed in 25 cases within 3 days after onset when the disease was at the initial stage. The small focal patchy opacity was found in 22 cases (88.0%, 22/25), and oval small ground-glass opacities in 20 cases (80.0%, 20/25) were among those cases. In another 35 cases, the first HRCT scans were performed 3 days later after onset when the disease was at the progressive stage. Multiple larger opacities were visualized in all cases at the progressive stage, they included the pure ground-glass opacities in 10 cases (16.7%, 10/60), ground-glass opacities accompanied by consolidations in 45 (75.0%, 45/60), prominent consolidations in 5 (8.3%, 5/60). The lesions strongly progressed in 50 cases (83.3%, 50/60) and distinctly resolved in 10 cases (16.7%, 10/60) within 10-14 days after admission. The pulmonary interstitial hyperplasia was found in 4 cases (6.7%, 4/60). Conclusion HRCT was able to distinctly demonstrate the density, shape, and range of pulmonary lesions of SARS, and reflect the alternation due to the interstitial hyperplasia of the lungs. HRCT can be used to display the early abnormalities and dynamic changes of SARS.
