颅内血管外皮细胞瘤与脑膜瘤的MR影像对照研究

【摘要】 目的 研究颅内血管外皮细胞瘤的MR影像表现 ,并与脑膜瘤进行对比分析。方法选取经手术及病理证实的颅内血管外皮细胞瘤 2 7例和脑膜瘤 2 36例。对 2组肿瘤的MR及临床观察指标进行单因素和多因素分析 ,统计学方法包括 χ2 检验、秩和检验和Logistic回归。 结果 患者性别 (χ2 =6 981 )、肿瘤形态 (χ2 =39 949)、肿瘤内血管流空信号 (χ2 =1 2 366)、肿瘤内坏死与囊变信号 (χ2 =37 432 )、肿瘤附着硬膜的类型 (广基底或窄基底 ) (χ2 =1 6 956)、T1 WI信号强度 (χ2 =1 4 880 )、T2 WI信号强度 (χ2 =37 0 68)、肿瘤增强程度 (χ2 =1 0 0 81 )、肿瘤增强均匀性 (χ2 =1 7 0 1 4 )等因素在血管外皮细胞瘤组与脑膜瘤组之间差异具有非常显著性意义 ,P值均 <0 0 1 ;“硬膜尾征”(χ2 =5 655)在两组之间差异具有显著性意义 ,P <0 0 5。患者初次就诊时年龄在两组间差异具有显著性意义 (双侧检验P =0 0 1 5) ,P <0 0 5 ;患者的病程在两组间差异具有非常显著性意义 (双侧检验P =0 0 1 3) ,P <0 0 5。Logistic回归方程中 ,变量“肿瘤形态”P <0 0 1 ;变量“T2 WI信号强度”和变量“硬膜尾征”P <0 0 5。结论 在颅内血管外皮细胞瘤与脑膜瘤的MR影像鉴别诊断诸多因素中 ,肿瘤呈分叶状、T2 WI呈等高混

【Abstract】 Objective To study the MRI findings in a series of intracranial hemangiopericytomas, and to compare these findings with those of intracranial meningiomas. Methods The MRI appearances and clinical features in 27 cases of intracranial hemangiopericytomas and 236 cases of intracranial meningiomas were retrospectively reviewed. All cases were proved by operation and pathology. The data were evaluated by using χ 2 test, rank sum test, and Logistic Regression. Results Sex (χ 2=6 981), shape of tumor (χ 2=39 949), signal voids of vessel in tumor (χ 2=12 366), necrosis or cystic degeneration in tumor (χ 2=37 432), the type of dural attachment ( broad based or narrow based ) (χ 2=16 956), T 1WI signal intensity (χ 2=14 880), T 2WI signal intensity (χ 2=37 068), enhanced degree of tumor (χ 2=10 081), homogeneous degree of tumor after contrast enhancement (χ 2=17 014), and duration time were significant different ( P <0 01) between hemangiopericytoma group and meningioma group. In addition, there was a significant difference ( P <0 05) between the two groups with "dural tail" sign (χ 2=5 655), duration time, and age of the first presentation ( t =2 502). In Logistic regression analysis, shape of tumor showed significant difference ( P <0 01) between the two groups. T 2WI signal intensity and "dural tail" sign also showed significant difference ( P <0 05) between the two groups in Logistic regression analysis. Conclusion Our data indicated that presence of the multilobulated tumor, iso and hyperintensity on T 2WI, and lack for "dural tail" sign were the most important characteristics for differentiating intracranial hemangiopericytoma from meningioma. It is helpful to get a correct diagnosis with other MRI and clinical features.
