核因子кB和诱导型一氧化氮合酶及白细胞介素6在鼻息肉组织中的表达

【摘要】 目的 探讨核因子кB(nuclerfactorкB ,NF кB )、诱导型一氧化氮合成酶 (induciblenitricoxidesynthases ,iNOS)和白细胞介素 6 (interleukin 6 ,IL 6 )在鼻息肉中的表达及意义。方法取鼻息肉组织 5 9例并以 2 0例鼻中隔偏曲患者下鼻甲 (inferiorturbinate ,IT)黏膜组织作对照 ,分别采用原位杂交方法检测NF кBmRNA ,应用免疫组化技术检测iNOS、IL 6的表达及分布。结果 ①NF кBmRNA、iNOS和IL 6主要定位于NP上皮细胞、间质中的炎性细胞、部分腺体和血管内皮细胞 ;②三者的阳性表达率 (5 4 2 4 %、6 2 71%和 6 4 4 1% )和表达强度均明显高于对照组 (10 %、2 0 %和 15 % ) ,且差异具有显著性 (P <0 0 1) ;③NP组织中存在着多量的嗜酸性粒细胞及嗜中性粒细胞的浸润明显高于IT组织 (P <0 0 1)。结论 ①NF кB、iNOS、IL 6可能参与了NP局部微环境的炎症反应 ;②嗜酸粒细胞及嗜中性粒细胞参与鼻息肉中炎症的发生与发展 ,以NF кB作为药物作用靶点 ,深入研究NF кB调控机制 ,可能有助于开拓新的炎症反应的治疗途径。

【Abstract】 Objective To evaluate the expression and significance of nuclear factor-к appaB(NF-кB)mRNA,inducible nitric oxide synthase(iNOS) and interleukin-6(IL-6) in the pathophysiology of nasal polyp(NP). Methods The specimens from 59 nasal polyps(NP) and 20 inferior turbinates (IT) were studied by in situ hybridization for NF-кB mRNA, and by immunohistochemistry for iNOS and IL-6. Tissue sections were observed under optical microscope.Results NF-кB mRNA, iNOS and IL-6 overexpressions were localized mainly to the nasal polyp epithelium, the various inflammatory cells, in submucosal endothelial cells around the vessel and in submucosal glands. The positive frequency of activation of NF-кB mRNA was higher in NP (54.24%) than in IT ( P <0.01), whereas the positive frequency of iNOS and IL-6 overexpression were higher in NP (62.71% and 64.41%) than in IT ( P <0.01).Eosinophils and neutrophils in NP were higher than in IT ( P <0.01). Conclusion These results suggest that NF-кB,iNOS and IL-6 are strongly correlated with the formation of NP. NF-кB,iNOS and IL-6 may act as anti-apoptotic molecules in eosinophils and neutrophils that may be responsible for the recruitment of inflammatory cells, particularly eosinophils, through the initiation of the transcriptional pathway of the related cytokines. NF-кB might play a key role in the pathogenesis of NP process and therefore is a potential target for medical treatment and new therapeutic intervention.
