急性胰腺炎血液炎症介质水平与心脏损伤关系的实验研究

【摘要】 目的  探讨急性胰腺炎时血液炎症介质水平与心脏损伤的关系。 方法  用猫自身胆汁加去氧胆酸钠以高、低两种注射压力 ( 2 0kPa和 3kPa)经胰管逆行注入 ,诱发猫产生重、轻两型急性胰腺炎模型。观察血内皮素 - 1(ET - 1)、肿瘤坏死因子 -α(TNF -α)、磷脂酶A2 (PLA2 )、肌酸激酶同工酶 (CK -MB)含量变化及胰腺、心肌的病理改变。 结果  两型急性胰腺炎ET - 1、TNF -α、PLA2水平明显高于对照组 (P <0 0 1)。各项指标重症组又明显高于轻症组 (P <0 0 1) ,ET - 1、TNF -α、PLA2水平与CK -MB的变化呈显著正相关 ,r分别为 0 816、0 6 96、0 6 6 8(P <0 0 1) ,并与急性胰腺炎致心脏病理改变相一致。 结论 ET - 1、TNF -α、PLA2可能参与了急性胰腺炎心脏损伤的发病过程。

【Abstract】 Objective To explore the effects of inflammatory mediators on heart damage in acute pancreatitis Methods The models of AP were made with the mixture solution(autologous bile mixtured with 30 mmol/L sodium deoxycholate,1∶1)being retrogradely injected into the pancreatic duct in different pressure.Twenty-eight mongrel cats were randomly divided into 3 groups:High pressure group(A,n=10),the injection pressure was 20 kPa for 5 minutes,then the pancreatic duct was ligated;Low pressure group(B,n=10),except with injection pressure 3 kPa,all other methods were the same as in A group;Control group(C,n=8):sham-operation,the pancreas was only moved aside.Blood endothelin-1(ET-1),tumor necrotizing factor-α(TNF-α),phospholipase A2(PLA2),creatine kinase isoenzyme(CK-MB)and histology of the pancreas and heart were observed after the models were made for 18h. Results The level of blood ET-1,PLA2,TNF-α increased significantly as compared with the control groupB( P<0 01 ),those in group A were higher than those in groupB( P<0 01 ).The levels of ET-1,TNF,PLA2 were positively related to CK-MB(r=0.816,0.696,0.668, P<0 01 ),and were associated with the degree of heart histologic alteration. Conclusion ET-1,TNF-α,PLA2 may take part in the process of heart damage in acute pancreatitis.
