卵巢交界性肿瘤细胞增殖活性与其临床病理间的关系

【摘要】 目的:探讨卵巢交界性肿瘤细胞增殖活性与其临床病理间的关系。方法:以WHO(1999年)卵巢肿瘤诊断标准,按照交界性卵巢肿瘤分化程度的不同,为其进行组织学分级,并采用免疫组化法对46例交界性卵巢肿瘤中增殖细胞核抗原(PCNA)的表达进行检测,同时利用流式细胞术(FCM)检测其DNA含量。结果:在交界性卵巢肿瘤中,高分化PCNA标记指数(20.5±12.17)、细胞核DNA含量DI(1.03±0.12)、PI(9.48±6.25)和异倍体率(12.50%)明显低于中低分化的PCNALI(52.35±27.25)、DI(1.20±0.14)、PI(20.34±9.78)和异倍体率(45.45%)(P<0.01),临床I期PCNA标记指数(15.4±11.46)、细胞核DNA含量:DI(1.04±0.11)、PI(12.06±7.68)和异倍体率(14.81%)明显低于Ⅱ、Ⅲ期的PCNALI(46.64±13.48)、DI(1.18±0.12)、PI(23.76±0.89)和异倍体率47.37%(P<0.01),但与其病理类型无明显相关性,(P>0.05)。结论:通过PCNA检测及流式细胞术(FCM)DNA含量分析可较准确地判定肿瘤的增殖潜能,并有可能成为交界性卵巢肿瘤患者预后的重要指标。

【Abstract】 Objective:To investigate the relationshi p betwee n clinical pathology and cellu-lar proliferative activity of ovarian borderline tumors.Methods:The histologic grade of ovarian borderline tumors were classif ied according to the differentiation grade diagnosesed with the WHO standard.Th e expression of proliferating cell nuclear antigen (PCNA)were detected in46c ases of ovarian borderline tumors by immunohistochemistry,and the DNA content were detected by flow cytometry.Results:The PCNA index(20.5±12.17) ,DNA content:DI(1.03±0.12),PI(9.48±6.25),and the heter oploid rate(12.5%)in the high-differentiated ovarian borderline tumors w ere lower than in low-differentiated tumors,PCNA LI(52.35±27.25) ,DI(1.20±0.14),PI(20.34±9.78),and the hetero-ploid ra te(45.45%)(P<0.01).The PCNA index(15.4±11.46),DNA c ontent:DI(1.04±0.11),PI(12.06±7.68),and the heteroploi d rate(14.81%)in clinical stage I ovarian borderline tumors were lower t han in stage and tumors,PCNA LI(46.64±13.48),DI(1.18±0.1 2),PI(23.76±10.89),and the het-eroploid rate (47.37%) (P<0.01).But there were no significant relationship within the patholo gic types.Conclusion:The detection of PCNA and the analysis of DNA content by FCM could deter-minate the proliferative potential of tumors exactly,would be an important prognostic marker of o-varian borderline tumors.
