【摘要】 目的　通过检测磁定位采集器采集的胃粘膜细胞涂片中端粒酶hTERT蛋白的表达 ,探讨其对胃癌诊断的价值。方法　采用抗人hTERT单克隆抗体 ,以免疫组化法检测通过磁定位采集器收集的 113例胃粘膜细胞涂片标本的hTERT蛋白表达。结果　 2 8例经组织病理学确诊的胃癌患者的涂片标本中 ,15例被细胞病理学诊断为癌 ,其中 14例hTERT阳性 ;2例被细胞病理学诊断为可疑癌的标本中 ,1例hTERT阳性 ;5例镜下查到不典型增生细胞的标本中 ,3例hTERT阳性 ;6例细胞病理学阴性的标本中 ,5例hTERT阳性。细胞涂片hTERT检测诊断胃癌的灵敏度为82 1%;细胞病理学诊断胃癌的灵敏度为 60 7%。同时 ,在 2 3例重度肠化标本中 ,测得 2例hTERT阳性 ,其余各良性病变组均为阴性。结论　磁定位采集胃粘膜细胞涂片标本的hTERT检测结合细胞病理学诊断可作为胃癌筛检和诊断的敏感而有效的方法。更多还原

【Abstract】 Objective Through detecting hTERT protein in exfoliated cells smear samples obtained from gastric mucosal cell by using the magnetic-medium sensory direction gastric mucosal cell collector,to probe its value in gastric carcinoma diagnosis. Methods Total of 113 exfoliated cells smear samples were prepared by using the magnetic-medium sensory direction gastric mucosal cell collector,and expression of human telomerase reverse transcriptase(hTERT)was analyzed by monoclonal antibody based-immunohistochemistry. Results 28 of 113 samples verified as gastric cancer by histopathology,hTERT protein was detected in 14 of 15 cytologic-positive specimens,1 of 2 cytologic-suspicious positive specimens,3 of 5 specimens with anaplastic cells,and 5 of 6 cytologic-negative specimens.2 of 23 specimens from patients with severe intestinal metaplasia were positive for hTERT.The rest of 113 specimens were all negative.The sensitivity of hTERT detection in exfoliated cells smear specimens to gastric carcinoma diagnosis was 82.1% and the specificity was 97.6%.Meanwhile,the sensitivity of cytologic confirmation was 60.7%. Conclusions The detection of telomeraes hTERT protein in exfoliated cells smear samples obtained from gastric mucosal cell using the magnetic-medium sensory direction gastric mucosal cell collector is a sensitive and specific method in screening and diagnosis of gastric carcinomas.更多还原
