脉压与冠心病危险的相关性研究

【摘要】 目的 :探讨人群脉压与冠心病危险的相关性。  方法 :对 2 5 5 3 7例高血压普查人群进行登记及测压 ,应用二值多元Logistic回归模型分析脉压与冠心病危险的相关关系。  结果 :人群总体脉压在 40~ 49mmHg(1mmHg =0 13 3kPa)时出现的频率最高 ,男女无显著差异 (P =0 0 5 9)。对血压以外的其他主要心血管病危险因素进行校正后 ,脉压每增加 10mmHg ,冠心病危险中年人增加 5 1 5 % ,老年人增加2 3 9%。但进一步校正舒张压后 ,老年人脉压与冠心病的正相关性不再具有统计学意义 ,进一步校正收缩压后 ,中、老年人脉压与冠心病的相关性均不再显著。  结论 :中、老年人脉压与冠心病虽然显著正相关 ,但其相关性不独立于收缩压和舒张压 ,在重视脉压的同时 ,应首先重视对收缩压和舒张压的控制。

【Abstract】 Objective: To explore the correlation between pulse pressure (PP) and coronary heart disease (CHD). Methods: A cross-sectional study was carried out in a natural population.Total 25 537 subjects were registered and their blood pressure was measured. The correlation between PP and CHD was analysized using binary logistic regression model. Results: The highest frequency of PP was at 40-49*!mmHg in the total population. There was no significant difference between sexes(p=0.059). With increment of 10 mmHg in PP, the CHD risk increased was by 51.5% in the middle-aged and 23.9% in the senile poupulation after adjusting other risk factors.After adjustment for diastolic pressure the correlation was not significant in the senile population, and for systolic pressure it was not significant in both groups. Conclusion: Though there is a significantly positive correlation between PP and CHD in middle-aged and senile population,the correlation is not independent of systolic and diastolic pressure. The levels of systolic and diastolic blood pressure should be taken great care of when CHD risk is evaluated.
