【摘要】 目的 :分析感染性心内膜炎的临床特征 ,比较研究国内、国际诊断标准。　　方法 :对 1989年至 2 0 0 2年经我院病理科诊断证实的 93例感染性心内膜炎患者进行临床特征分析 ,并以此为依据比较两种诊断标准的敏感性。　　结果 :连续 2次或 2次以上血培养阳性 ,且为相同致病菌的有 2 0例 ,血培养阳性 1次的有 8例。超声心动图明确发现赘生物 71例 ,脓肿 3例 ,人工瓣膜部分开裂 3例。发现主要血管征象 2 5例。新出现反流杂音或杂音加重 2 8例。中国标准的敏感性为 76% ,Duke标准敏感性为 43 % (P <0 0 0 1)。　　结论 :中国标准敏感性明显高于Duke标准 ,但仍需进行更多临床研究 ,特别是有关特异性临床研究。更多还原

【Abstract】 Objective: To analyze the clinical features and compare the domestic with international diagnostic criteria of infective endocarditis(IE). Methods: The clinical features of 93 cases of pathologically confirmed IE were analyzed. Domestic and international diagnostic criteria for IE were compared and assessed according to the obtained results. Results: Positive results of the same germ cultured for two or more times were identified in 20 cases and once in 8 cases. Echocardiography demonstrated vegetations in 71, abscesses in 3 and new prosthetic regurgitation in 3. Major vascular phenomena were discovered in 25. New valvular regurgitation and murmur were found in 28. The sensitivity of domestic criteria was 76%,but Duke’s was 43%(p<0.001). Conclusion: Domestic criteria are superior to the Duke’s in the diagnosis of IE.More clinical trials are needed to be studied,especially those about the specificity of domestic criteria.更多还原
