口服葡萄糖耐量试验与馒头餐试验对2型糖尿病病人血糖代谢及酮症发生的影响
The comparative study of effects of OGTT and bread meal test on the risks of ketonuria and blood glucose metabolism in type 2 diabetic patients
【摘要】 目的 了解口服葡萄糖耐量试验 (OGTT)和馒头餐两种检查在已确诊的 2型糖尿病病人中对酮症发生的危险性和血糖代谢的影响。 方法 选取住院的 2型糖尿病病人 2 0 0例 ,随机分为两组 ,每组 1 0 0例 ,分别进行 OGTT或馒头试验餐检查 ,观察两种检查方法后的酮症发生率、持续时间及检查后 2 4 h内血糖的波动情况。 结果 (1 )共有 5例病人行上述检查后尿酮体阳性 ,OGTT组2例 (2 % ) ,馒头餐组 3例 (3% )。两组间酮症的发生率差异无显著意义 (P=0 .5 6 )。 5例尿酮体阳性病人均于午餐时恢复服降糖药物治疗后尿酮体转阴 ,无不适主诉。(2 )检查后 2 4 h,两组间的血糖平均水平相似。 结论 OGTT和馒头餐检查对 2型糖尿病病人血糖代谢影响相似 ,导致病人酮症发生率相似。 OGTT仍可作为评价 2型糖尿病病人胰岛功能的检查方法
【Abstract】 Objective To compare the effects of OGTT and bread meal test (equivalent to 75 g of glucose) on the risks for ketonuria and blood glucose metabolism in type 2 diabetic patients. Methods 200 admitted type 2 diabetic patients were randomly divided into two groups in which OGTT and bread meal test were carried out separately. There were 100 patients in each group. Incidence and duration of ketonuria, as well as blood glucose change within 24 hours of the test were observed. Results (1) There were five patients with ketonuria after the test, two in OGTT group(2%), three in bread meal test group(3%). There was not difference in the incidence of ketonuria between the two groups. The ketonuria in the five patients disappeared after resumption of regular therapy. (2) No difference in glucose level between two groups was found within 24 hours of the tests. Conclusion Compared with the bread meal test, OGTT does not confer more risk of ketonuria and not cause higher glucose level. Therefore OGTT test can be used as a regular test in studying β cell function and glucose metabolism in type 2 diabetes.
