高血压患者eNOS基因变异与饮酒间交互作用

【摘要】 目的 探讨内皮型一氧化氮合成酶 (eNOS)基因第 7外显子G894T(Glu2 98Asp)变异与饮酒之间交互作用在高血压患病中的意义。方法 以某工厂人群中筛检出的、未经药物系统治疗的 116名高血压患者及 136名血压正常者为研究对象 ;运用PCR -限制性片段长度多态性检测G894T变异 ;用相加模型分析G894T变异与饮酒之间的交互作用 ;用人群归因危险度百分比计算人群中的病因分值。结果 G894T变异与饮酒之间对高血压患病具有正交互作用 ;归因交互效应为 3 6 9;归因交互效应百分比为 4 8 6 8% ;纯因子间归因交互效应百分比为 5 6 0 8%。用多元Logistic回归调整年龄、性别、吸烟、体重指数后 ,G894T变异与饮酒之间对高血压患病仍具有正交互作用。调整上述混杂因素后 ,归因交互效应为 1 13;归因交互效应百分比为 19 0 9% ;纯因子间归因交互效应百分比为 2 2 97%。在给定条件下计算的人群归因危险度百分比约为 10 %~ 15 %。结论 eNOS基因G894T变异与饮酒之间交互作用在该研究人群高血压患病中具有重要意义 ;在仅仅对具有G894T变异与饮酒同时存在的这一小部分人群中控制饮酒可明显降低一般人群高血压患病危险。

【Abstract】 Objective To analyze the interaction between G894T(Glu298Asp) mutation in the exon 7 of endothelial nitric oxide synthase gene and alcohol drinking in essential hypertension.Methods One hundred and sixteen essential hypertensives,screened from a steel factory workers, not taking hypertensive medication,and one hundred and thirty six normotensives,screened from health workers in the same factory were selected as the subjects in this study.Polymerase chain reaction(PCR) and restriction fragment length polymorphism(RFLP) were performed to detect the G894T mutation;Additive model was used to analyze the interaction;Population attributable risk percent(PAR%) was applied to calculate the etiologic fraction.Results There was a positive interaction between G894T mutation and alcohol drinking in essential hypertension;Attributable interaction was 3.69;Attributable interaction percent was 48.68%;Pure attributable interaction percent was 56 08%.After adjusting age,sex;smoking,body mass index by logistic regression, there was also positive interaction between G894T mutation and alcohol drinking in essential hypertension;Attributable interaction was 1.13;Attributable interaction percent was 19.09%;Pure attributable interaction percent was 22.97%.Under certain conditions,PAR% was about 10%-15%.Conclusions The interaction between G894T mutation in the exon 7 of endothelial nitric oxide synthase gene and alcohol drinking had a positive effect on the essential hypertension in this studied population.To control the alcohol drinking in the population who had G894T mutation and alcohol drinking simultaneously would produce markedly decrease of the essential hypertension in general population.
