椎间盘镜技术同种异体骨移植治疗骨不连及骨缺损的临床研究

【摘要】 目的 探讨椎间盘镜技术同种异体骨移植治疗骨不连及骨缺损的可行性。方法 硬膜外或局部麻醉 ,先用C型臂X线机定位 ,用椎间盘镜的导针经皮穿刺到骨不连间隙内 ,然后行 2 0cm的小切口 ,并用套筒逐步扩大 ,置入椎间盘镜 ,在电视监控下剥离周围骨膜 ,清除骨不连间隙内纤维组织 ,打通两端髓腔 ,植入成品同种异体冻干骨小块 ,取出间盘镜、缝合小切口、弹力绷带加压包扎。结果  1 3例病人手术后 5、6、8、1 2个月临床愈合 ,平均愈合时间 7 5个月 ,骨愈合率 1 0 0 %。结论 椎间盘镜技术应用到四肢骨折及骨不连的植骨扩大了椎间盘镜的用途 ,该方法最突出的优点是创伤小 ,方法简便易行 ,随着部分器械的进一步改进 ,椎间盘镜下同种异体骨移植治疗骨不连必将取得肯定疗效 ,有待于进一步推广

【Abstract】 Objective To investigate the feasibility for intervertebral disc scope and homologous bone transplantation to treat nonunion of fractures Methods Intervertebral disc scope was inserted into the space of nonunion under fluoroscopic guidance.The periostem and soft tissue around the nonunion were stripped.Fibrous tissue within the nonunion was removed and the medullar cavity at the both ends was got through.Small pieces of freeze dried homologous bone were implanted.Results The nonunion in 13 patients were cured after 5,6,8,12 months.The average healing time was 7 5 months.The healing rate was 100%.Conclusion The technique of intervertebral disc scope to treat the nonunion of defect of the extremity bone enlarges the application of the intervertebral disc scope and offers a new method to treat the nonunion or defect of the extremity bone.The advantages of the technique are as follows:minor surgical trauma,operation with the limit of the local skin scar,the reliable result of bone transplantation.So it is worthy of application and popularization.
