【摘要】 目的 :探讨体表超声对胃癌浸润深度诊断价值。方法 :为前瞻性研究。对 15 9例胃癌术前行体表超声检查 ,并与手术及术后病理结果比较。结果 :超声对胃癌浸润深度总诊断符合率为 74 .2 % ,对 T1、T2、T3和 T4诊断符合率分别为 5 3.8%、 71.4 %、 84 .1%和 6 7.8% ,T1诊断率最低 ;浸润深度过深判断与肿瘤伴炎症、溃疡疤痕形成有关。超声对肝脏及胰腺受侵诊断符合率较高 ,分别达 80 .0 %及 73.5 % ;十二指肠、横结肠、脾及脾门受侵诊断符合率较低 ,膈肌受侵超声不易诊断。病变厚度及长度与肿瘤浸润深度有一定关系。结论 :体表超声有助于判断胃癌浸润深度 ,可成为胃癌术前分期常规检查方法。更多还原

【Abstract】 Objective:To study the value of surface ultrasonography in diagnosing the depth of gastric cancer invasion.Methods:One hundred fifty-nine cases with gastric cancer were preoperatively examined by surface ultrasonography, and the results were compared with the operative and pathological findings.Results:The overall accuracy for the depth of cancer invasion was 74.2%. The sensitivity for T1?T2?T3 and T4 was 53.8%?71.4%?84.1% and 67.8%,respectively. Ultrasonography overestimation of the depth may be caused by the presence of inflammatory cell infiltration and ulcer scarring. The sensitivity for diagnosing invasived liver and invasived pancreas was 80.0% and 73.5%, respectively. It is difficult to diagnose invasived diaphram. The thickness and extent of cancer were associated with the depth of invasion.Conclusions:Surface ultrasonography is useful in evaluating the depth of gastric cancer invasion, and it may be a routine method for preoperative staging.更多还原
