齿接触半环抱钢板对骨折愈合影响的动物实验

【摘要】 目的观察齿接触半环抱钢板内固定对骨折愈合的影响。方法成年新西兰大白兔30只,体重2.8~3.5公斤。双侧胫骨中段横形截骨,右侧用齿接触半环抱钢板内固定(实验组),左侧用同等型号的AO传统钢板内固定(对照组)。术后不同时期处死实验动物,获取骨标本分别行X-Ray、光镜、透射电镜观察。结果X-Ray:实验组骨痂分布更为均匀、有序,骨髓腔再通较早。光镜观察:对照组钢板下皮质骨骨吸收较早出现,并呈进行性发展;实验组钢板下第2~4周有大量骨痂生成,第6周改建成与骨纵轴平行。电镜观察:对照组钢板下皮质骨中大多数骨细胞不完整,细胞器肿胀,并呈进行性发展。实验组结构完整,细胞器稍肿胀,并呈进行性改善。结论齿接触半环抱钢板对骨折愈合的环境干扰小,有利于骨折愈合。

【Abstract】 Objective To compare the effects of fixation with the tooth contact half cycle (TCC) steel plate and those of the similar pattern AO conventional steel plate on fracture union. Methods 30 adult New Zealand rabbits, weighed from 2.8~3.5 kilogram, were used in this experiment. After the transverse osteotomy of middle shaft of both tibiae was operated on them, internal fixation with TCC steel plate was done in the right tibiae (the experimental group) and that with the similar pattern AO conventional steel plate in the left (the control group). The rabbits were killed respectively at different stages after surgery, and bone samples were harvested and prepared for X Ray, light microscopy, electron microscopy observation. Results X Ray observation:The distribution of bony callus in the experimental group was more even and tidy than that in the control group. And the medullary cavity of bone was reformed earlier in the experimental group. Light microscopy observation:In the control group, bone absorption of the cortical bone beneath the steel plate was seen at an early stage of internal fixation and developed with time, while in the experimental group, a large quantity of bone callus was seen from two to four weeks,and the reconstruction of bony callus was parallel to the bone longitudinal axis at six weeks postoperatively. Electron microscopy observation:In the control group, most osteoblast cells in the cortical bone beneath the steel plate were not intact and organelles swelled severely and developed with time, while in the experimental group, all osteoblast cells were intact, and organelles swelled slightly and ameliorated with time. Conclusion The fixation with TCC steel plate only slightly interferes with the fracture union environment.
