【摘要】 目的 :　了解腹膜透析患者左心室肥厚与炎症反应、营养不良之间的相关关系。方法 :　用超声心动图了解 46例腹膜透析 (CAPD)患者、1 8例肾衰竭未行透析患者 (CRF)及 1 6例健康人 (Cont)左心室肥厚 (LVH)的发病率 ,检查血肿瘤坏死因子 - α(TNF- α)、以及白蛋白水平 ,并用主观整体评价 (SGA)法评价三组对象的营养状态。结果 :　按 SGA标准 ,腹膜透析组、肾衰竭组的营养状况与对照组相比有明显差别 (P<0 .0 5 )。CAPD组、肾衰竭组和对照组的血白蛋白水平、炎症因子水平均有显著性差异 (P<0 .0 0 1 )。 SGA为 C级的患者血白蛋白水平与 A、B级的血白蛋白有显著性差异 (P<0 .0 5 )。 TNF-α与白蛋白之间呈明显负相关关系 (r=- 0 .2 4 3 ,P<0 .0 5 ) ,与LVH之间呈明显正相关关系 (r=0 .5 2 6 ,P<0 .0 5 )。结论 :　透析患者普遍存在炎症反应增高和营养不良 ,营养不良的患者炎症因子的水平也明显增高 ,同时与左心室肥厚发生率存在正相关关系。更多还原

【Abstract】 Objective: To investigate the relationship between LVH, inflammation and malnutrition in dialysis patients through cross-sectional study.Methods: Forty six CAPD and eighteen CRF were included in this study, and sixteen healthy adults served as control. The presence of LVH was determined by ultrasonic cardiography(UCG). Plasma TNF-α?IL-6 and albumin were examined. Their nutritional status were measured using SGA. Results: According to SGA, the nutritional status of dialysis and CRF patients had significant difference with normal (P<0.05). The plasma albumin and inflammatory level had significant difference in CAPD patients compared to CRF and control group(P<0.001), and the albumin level was much lower in C according to SGA than those in A and B. TNF-α had significant correlation with albumin (P<0.05), and it also had strong correlation between inflammation and LVH(P<0.05).Conclusion: Malnutrition and inflammation in dialysis patients are prevelant and it has strong positive correlation with the incidence of LVH.更多还原
